BACKGROUND. Cranio-cervical mandibular disorders, also known as Temporomandibular Disorders or TMDs, are a complex of painful and dysfunctional conditions affecting the skull, the Temporomandibular Joints (TMJ) and the cervical spine. The evaluation of these disorders was based on Diagnostic Criteria for Temporomandibular Disorders (DC / TMD), published in the literature and recognized internationally, which contemplate two axes of evaluation: Axis I (physical evaluation) and Axis II (evaluation of pain, parafunctions and psychological state). OBJECTIVE. Identify and standardize the main characteristics of an evaluation sheet that makes it possible to make specific physiotherapy diagnoses of TMDs and that at the same time makes the evaluation of cranio-cervical-mandibular disorders simple and appropriate. MATERIALS AND METHODS. The evaluation form drawn up by Dr. Bizzarri was adopted: the Bizzarri protocol. In the S.O.D. Maxillofacial Surgery of Ospedale Riuniti di Torrette a descriptive observational study was conducted according to this protocol. For the success of the therapeutic path, informative and official material on narrative medicine was also sought, with patient centered communication and patient history, to be used for the benefit of the relationship between therapist and patient. RESULTS. Dr. Bizzarri's evaluation form appears to have a real diagnostic appropriateness, because it is correlated with the international DC / TMD criteria. CONCLUSIONS. The best diagnostic criteria of the DTMs were identified and reviewed for an optimal understanding of the evaluation process, taking into account the patient centered communication model and the patient's history.
BACKGROUND. I disordini cranio cervico mandibolari, conosciuti anche come Disordini Temporomandibolari o TMD, sono quel complesso di condizioni dolorose e disfunzionali che interessano il cranio, le Articolazioni Temporomandibolari (ATM) e del rachide cervicale. La valutazione di questi disordini si è fondata su Criteri Diagnostici per i Disordini Temporomandibolari (DC/TMD), pubblicati in letteratura e riconosciuti a livello internazionale, che contemplano due assi di valutazione: Asse I (valutazione fisica) e Asse II (valutazione del dolore, delle parafunzioni e dello stato psicologico). OBIETTIVO. Individuare e standardizzare le principali caratteristiche di una scheda valutativa che renda possibile fare diagnosi fisioterapiche specifiche dei DTM e che allo stesso tempo renda semplice ed appropriata la valutazione dei disordini cranio cervico mandibolari. MATERIALI E METODI. È stata adottata la scheda di valutazione redatta dal dottor Bizzarri: il protocollo Bizzarri. Nella S.O.D. Chirurgia Maxillo Facciale di Ospedale Riuniti di Torrette è stato condotto uno studio osservazionale descrittivo secondo questo protocollo. Per una buona riuscita del percorso terapeutico è stato ricercato anche materiale informativo e ufficiale sulla medicina narrativa, con la comunicazione patient centered e la storia del paziente, da utilizzare a beneficio del rapporto tra terapista e paziente. RISULTATI. La scheda valutativa del dottor Bizzarri risulta avere una vera e propria appropriatezza diagnostica, perché correlata con i criteri internazionali DC/TMD. CONCLUSIONI. Sono state individuati e revisionati quelli che sono i migliori criteri diagnostici dei DTM per un’ottimale comprensione del processo valutativo, tenendo conto del modello di comunicazione patient centered e della storia del paziente.
