From its first introduction in clinical practice, the many advantages of PICC, have determined a crescent use of the device. becase of its nature of central venous access, it allows the infusion of drugs that couldn't be given by peripheral injection, ensuring much lower risk of complications than the CVC, both reguarding the inserting procedure and the permanence of the device and its management. For what concernes expenses, PICC insertion, requires lower costs and it can be positioned by secifically trained nurses. Despite verified benefits, the use of PICC could lead to complicances that represent a serious threath for the patients health. This can happen at every moment: during the inserting procedure, as well as in management and handling of the device by nurses, and because of the approach and coexistence of the patient with the PICC, especially when the patient is at home. However theese issues could be significantly reduced or even zeroed out through the application of evidence based guidelines. In this paper recent scientific studies have been consulted concerning nursing approach in improvement of conditions of PICC patients and reports proposing new methods to increase patients' complince, because clinical practice has shown how standard tecniques of information and education are often inadeguate. The main reference for this work are three reports suggesting respectively the use of apps like 'FaceTime', multimedial tools like videos and powerpoint presentation and the utilization of cognitive-behavioural therapy along with the app 'WeChat'. Specifically it appears that the main goal has been widely achieved, indeed, many studies, seems to give ad adeguate sanitary education to users, assuring an efficent follow-up system that limits early and late complicances and increases patients' soddisfaction levels.
Dalla sua introduzione nella pratica clinica il PICC ha riscosso sempre più successo per via dei suoi numerosi vantaggi. Essendo un accesso venoso centrale a tutti gli effetti consente l’infusione di farmaci che non potrebbero essere somministrati per via periferica, garantendo un rischio di complicanze molto inferiore rispetto all’impiego dei CVC, sia per quanto riguarda la procedura di inserimento, sia durante tutta la permanenza del device e la sua gestione. Anche sul fronte economico la sua applicazione richiede costi inferiori e può essere posizionato da infermieri che hanno svolto un apposito corso di formazione. Nonostante i comprovati benefici, l’utilizzo del PICC non è privo di complicanze, che costituiscono una grave minaccia per la salute del paziente. Queste possono verificarsi in ogni momento: durante la procedura di inserimento, nella gestione e manipolazione del dispositivo ad opera degli infermieri e nell’approccio e nella convivenza del paziente con il PICC, specialmente a livello domiciliare. Tuttavia queste criticità possono essere notevolmente ridotte o addirittura azzerate attraverso l’osservazione di linee guida evidence based. Sono stati consultati studi scientifici recenti per quanto concerne il miglioramento del nursing rivolto ai pazienti con accesso venoso centrale ad inserzione periferica e pubblicazioni che propongono nuovi approcci per aumentare la compliance del paziente, poiché la pratica clinica mostra come le tecniche standard di informazione ed educazione spesso non sono sufficienti. Sono quindi stati presi come riferimento tre articoli, che rispettivamente suggeriscono l’impiego di applicazioni come ‘FaceTime’, strumenti multimediali come video e presentazioni PowerPoint e l’utilizzo della terapia cognitivo comportamentale insieme all’applicazione ‘WeChat’. In particolare l’obiettivo principale risulta essere ampiamente raggiunto. Infatti gli studi sembrano fornire un’adeguata educazione sanitaria agli utenti, garantendo un efficace sistema di follow-up in modo da limitare le complicanze precoci e tardive e migliorare il livello di soddisfazione del paziente.
