ABSTRACT Introduction Malnutrition is a relevant issue in patients with advanced oncological pathology to the point that we can speak of “disease within disease”. It is related both to antineoplastic treatments and to tumor itself. Therefore it is essential to implement screening systems for an early recognition of a condition of malnutrition or the risk of developing it, also by reason of the close link with the quality of life of these patients. Objectives General objective: to evaluate the impact of nutrition on the quality of life in oncological patients in an advanced state of the disease. Specific objectives: to highlight the evidence based interventions for the evaluation and management of nutrition useful for the health care planning. Matetials and methods Starting from the raccommendations provided by the ESPEN guidelines and the Guidelines on nutritional interventions in oncological patient provided by the Ministry of Health and the Nutritional Academy document “Clinical nutrition in oncological patient: meeting needs, guaranteeing pathways” a review was carried out of the scientific literature by consulting the Medline and Cochrane database, relative to the specific aspects of nutrition in the advanced oncological patient. Result There is a close relationship between nutritional status and quality of life in the advanced oncological patient. After an accurate evuation of the medical case, life expectancy and preferences of the patient it is advisable to choose the most suitable nutritional support. Conclusions The review carried out has made it possible to deepen an aspect of nursing care for oncological patients with advanced disease that is as important as underestimated and has opened the door to new perspectives of interventions that take into account cultural, psychosocial and multimodal aspects, in order to guarantee an individual assistance and of high quality. ABSTRACT Introduzione La malnutrizione è una problematica rilevante nei pazienti con patologia oncologica avanzata tanto che si può parlare di “malattia nella malattia”. Essa è correlata sia ai trattamenti antineoplastici, sia al tumore stesso. È quindi fondamentale mettere in atto sistemi di screening per un riconoscimento precoce di uno stato di malnutrizione o del rischio di svilupparlo, anche in virtù dello stretto legame con la qualità di vita di tali pazienti. Obiettivi Obiettivo generale: valutare l’impatto della nutrizione sulla qualità della vita nei pazienti oncologici in stadio avanzato di malattia Obiettivi specifici: Evidenziare gli interventi evidence based di valutazione e gestione della nutrizione utili alla pianificazione assistenziale Materiali e metodi A partire dalle raccomandazioni fornite dalle linee guida ESPEN e dalle Linee Guida sugli interventi nutrizionali nel paziente oncologico fornite dal Ministero della Salute e al documento della Nutritional Academy “La nutrizione clinica nel paziente oncologico: soddisfare i bisogni, garantire i percorsi” è stata effettuata una revisione della letteratura scientifica consultando le banche dati Medline e Cochrane, relativamente agli aspetti specifici della nutrizione nel paziente oncologico in fase avanzata Risultati Esiste una stretta relazione tra stato nutrizionale e qualità della vita nel paziente oncologico in fase avanzata. Dopo un’accurata valutazione del quadro clinico, dell’aspettativa di vita e delle preferenze del paziente è indicato scegliere il più appropriato supporto nutrizionale. Conclusioni La revisione condotta ha permesso di approfondire un aspetto dell’assistenza infermieristica ai pazienti oncologici con malattia avanzata tanto importante quanto sottovalutato e ha aperto le porte a nuove prospettive di interventi che tengano in considerazione gli aspetti culturale, psicosociale e multimodale, al fine di garantire un’assistenza individualizzata e di qualità.
