Vacuum Assisted Closure (V.A.C ..) is an advanced and non-invasive treatment for wound management, which consists in the application of sub-atmospheric pressure through a sophisticated, sterile and dreaded hermetic dressing, whose properties create a moist environment, at the wound site. The development of therapy a negative pressure positively influenced the management of both difficult and acute wounds than chronic. This technique allows to reduce the healing time of the carrying wounds an improvement in the patient's life both in terms of reduction of associated risks to chronic wounds, both by improving the psychosocial state of the patient. Objectives: The objective of this paper is to understand whether therapy a negative pressure can be a valid, effective and affordable treatment for cure difficult wounds and whether it is specifically manageable in the home setting. A narrative review of the literature was carried out, in which they were included recommendations and guidelines relating to the use of the NWPT, found by various engines research, such as Pubmed, Cochrane Library, Google Scholar. From what is reported in the literature, it emerges that the NPWT (negative pressure wound therapy) is found to be beneficial in economic terms, nursing care and healing time. Regarding the use of NPWT at home, it emerged as home treatment turns out to be more effective, as the patient, feeling at ease, has a greater adherence to treatment and improve self-care. It can be confirmed that NWPT appears to be a favorable treatment for the treatment of difficult wounds of various nature and etiology, especially in the home setting. In addition to this, it was possible to see how the use of a holistic approach is one adequate health education for the patient or family members increases compliance and well-being of the person, and that the implementation of specific programs for the setting home care would be a good response to this growing need for care. La Vacuum Assisted Closure (V.A.C..) è un trattamento avanzato e non invasivo per la gestione delle ferite, che consiste nell’applicazione di una pressione sub-atmosferica tramite una medicazione sofisticata, sterile e a temuta ermetica, le cui proprietà creano un ambiente umido, in corrispondenza del sito della ferita. Lo sviluppo della terapia a pressione negativa ha influenzato positivamente la gestione delle ferite difficili sia acute che croniche. Questa tecnica permette di ridurre i tempi di guarigione delle ferite portando un miglioramento nella vita del paziente sia in termini di riduzione dei rischi associati alle ferite croniche, sia migliorando lo stato psicosociale del paziente. Obiettivi: L’obiettivo di questo elaborato consiste nel comprendere se la terapia a pressione negativa possa essere un trattamento valido, efficace e conveniente per la cura delle ferite difficili e se sia gestibile nello specifico nel setting domiciliare. È stata effettuata una revisione narrativa della letteratura, nella quale sono state incluse raccomandazioni e linee guida inerenti all’utilizzo della NWPT, reperite da diversi motori di ricerca, quali Pubmed, Cochrane Library, Google Scholar. Da quanto riportato nella letteratura, emerge che la NPWT (negative pressure wound therapy) risulti essere vantaggiosa in termini economici, di assistenza infermieristica e di tempo di guarigione. Riguardo l’utilizzo della NPWT a domicilio, è emerso come il trattamento a domicilio risulti essere più efficace, in quanto il paziente, sentendosi a proprio agio, ha una maggior aderenza al trattamento e migliori la cura di sé. Si può confermare che la NWPT risulti essere un trattamento favorevole per la cura delle ferite difficili di varia natura ed eziologia, soprattutto in ambito domiciliare. Oltre a questo, si è potuto constatare come l’utilizzo di un approccio olistico e una adeguata educazione sanitaria al paziente o ai familiari aumenti la compliance e il benessere della persona, e che l’implementazione di programmi specifici per il setting domiciliare sarebbe una buona risposta a questo crescente bisogno assistenziale.
