Ruolo dell’infermiere nella gestione del paziente traumatizzato dalla chiamata al 118 al trattamento intraospedaliero: una revisione della letteratura

Introduction: Trauma is the leading cause of death affecting the population between the ages of 1 and 40, contributing drastically to determining a high number of permanent disabilities and increasing social costs. According to international guidelines, for the correct management of multiple-traumatized patients, an intervention in a short time (golden hour and few minutes platinum), transport of the patient to the most suitable and best equipped hospital and, above all, adequate training and training of rescue personnel. Objective: The objective of my paper is to describe and address, through a review of the literature, the role of the nurse in the emergency setting in the management of the polytrauma patient and the role that this professional plays in the 118 Operations Centre, in the place of the accident (outside the hospital) and in the intra-hospital setting. Results: The research showed that the emergency nurse, as explained in the DPR 27/03/92, covers greater autonomy and a role with greater centrality than what is foreseen in the job description in which the nurse could only perform well-defined tasks and under medical clearance. Conclusion: From the literature cited, it can be observed how essential it is for the nurse to possess specific knowledge in the traumatological field. You must know the effects of the trauma on the patient, the importance of the time factor and you must know how to act promptly by working as a team and collaborating with the doctor.

Introduzione: Il trauma è la prima causa di morte che colpisce la popolazione tra gli 1 e 40 anni, contribuendo in maniera drastica a determinare un elevato numero di invalidità permanenti e l'aumento del costo sociale. Per la corretta gestione del paziente politraumatizzato è indicato, secondo le linee guide internazionali, un intervento in breve tempo (golden hour e few minutes platinum), il trasporto del paziente all’ospedale più adeguato e più attrezzato e, soprattutto, un adeguato addestramento e preparazione del personale di soccorso. Obiettivo: L’obiettivo del mio elaborato è quello di descrivere e di affrontare, tramite una revisione della letteratura, il ruolo dell’infermiere in ambito di emergenza nella gestione del paziente politraumatizzato ed il ruolo che riveste tale professionista nella Centrale Operativa del 118, nel luogo dell’incidente (extraospedaliero) ed in ambito intraospedaliero. Risultati: Dalla ricerca è emerso che l’infermiere in emergenza, come esplicitato nel DPR 27/03/92, ricopre maggior autonomia ed un ruolo con maggior centralità rispetto a quanto previsto nel Mansionario nel quale l’infermiere poteva eseguire solamente compiti ben definiti e sotto autorizzazione medica. Conclusione: Dalla letteratura citata, si osserva quanto sia fondamentale per l’infermiere possedere conoscenze specifiche in campo traumatologico. Deve conoscere gli effetti del trauma sul paziente, l’importanza del fattore tempo e deve saper agire tempestivamente lavorando in equipe e collaborando con il medico.
