ABSTRACT Introduction The management of pressure ulcers is extremely fragmentary, where the nursing staff make a subjective assessment that varies according to their personal experience. For this thesis, I chose to present a retrospective study of the case of a patient being treated for pressure ulcers at the time of my internship at the clinic. Objective The objective of this paper is to focus on the continuity of care guaranteed by formal and informal resources and by the professionalism and competence of healthcare workers. Materials and methods I reviewed the treatment plan of a patient with pressure ulcers to achieve the objectives established in my experimental thesis. The plan was created by the wound care nurse specialist that followed me during my internship. Results The patient was taken into care on 02/08/2022 and completed the course of treatment on 10/03/2023. Seven months were necessary to complete the healing process. Discussion The treatment plan demonstrated that continuity of care is understood both as continuity between the various professionals integrated into a single framework, and as continuity between the different levels of assistance. The wound care nurse specialist takes on a very important role, not only as regards assistance but also in the development of the profession itself. Conclusions This work achieved its expected objective by highlighting how formal and informal resources contributed to ensuring continuity of care, which is centered on the individual patient and guided by care protocols delivered by a multidisciplinary team.
ABSTRACT Introduzione La gestione delle lesioni da pressione risulta estremamente frammentaria, dove il personale infermieristico si avvale di una valutazione soggettiva, diversa a seconda del proprio bagaglio personale. Per questo lavoro di tesi ho scelto di presentare uno studio retrospettivo di un caso di un paziente portatore di lesioni da pressione, seguito in ambulatorio, rifacendomi alla mia esperienza di tirocinio. Obiettivo L’obiettivo di questo elaborato è porre l’attenzione sulla continuità assistenziale garantita dalle risorse formali ed informali e dalla professionalità e competenza degli operatori sanitari. Materiali e metodi Per il raggiungimento degli obiettivi stabiliti dalla mia tesi sperimentale ho visionato il piano di trattamento di un paziente affetto da lesioni da pressione, realizzato dall’infermiera specializzata in wound care, che mi ha seguita durante il tirocinio. Risultati Il paziente è stato preso in carico il 02/08/2022 e ha terminato il percorso di cura il 10/03/2023. Per la completa guarigione si sono resi necessari 7 mesi di trattamento. Discussione Dal piano di trattamento attuato, è emerso che la continuità delle cure è intesa sia come continuità tra i diversi professionisti integrati in un quadro unitario, sia come continuità tra i diversi livelli di assistenza. L’infermiere esperto in wound care può giocare un ruolo importantissimo non solo nell’assistenza, ma anche nello sviluppo della professione in sé. Conclusioni Questo lavoro ha raggiunto l’obiettivo previsto evidenziando come le risorse formali e le risorse informali hanno contribuito a garantire la continuità dell’assistenza, in quanto questa è centrata sul singolo paziente, guidata da protocolli assistenziali ed erogata da parte di un team multidisciplinare.
