Backgroung Neonatal Substance Abstinence Syndrome (SAN) correlates with different critical issues both clinical-care and social-health issues; for these reasons, as highlighted in the literature, nurses can often find themselves emotionally overinvolved in the taking care of these infants to the point of compromising the quality of care provided. The aim of the study is to investigate the nurse's hemorrhagic experience and the any care implications encountered. Materials and methods Qualitative phenomenological study using Cohen's method conducted through interviews semi-structured interviews with some nurses belonging to the Departmental Organizational Structure (S.O.D.) of Neonatology of the Azienda Ospedaliero Universitaria delle Marche, chosen with intentional sampling. Data collection was followed by transcription verbatim of the interviews, which were then coded and, once the codes were identified, i.e., key words, they were placed within categories to facilitate the analysis thematic. A follow-up of the themes detected was conducted with the study participants. Results The sample analyzed (n=5 nurses) was characterized by strong heterogeneity in the responses, from which four main thematic areas emerged and were addressed: experience in the operational setting, which proves to be a fundamental tool nursing for care (I), the critical issues and difficulties faced by nurses during the hospitalization of the newborn with SAN (II), the emotions experienced by staff nurses (III), and the quality of care delivered (IV). Analysis and discussion Analysis of the interviews about the emotional experience of nurses confirms the difficulties caring with which they interface during the period of the newborn's stay in the hospital. Thus, not only does it highlight the presence of critical clinical-assistance issues in the care of an infant with SAN, but also problems in the building a therapeutic relationship with the parent, particularly with the mother addict, who can often become the target of accusations, prejudice and stigmatization by health professionals as widely discussed in the literature. Conclusions In conclusion, despite the clinical and care complexity of the infant with SAN, the quality of care is positively affected by the feeling of affection that nurses feel toward him or her; however, it highlights the need for specific training on the topic of pathological dependence in the newborn.
Introduzione La Sindrome di Astinenza Neonatale da sostanze (SAN) si correla a differenti criticità sia di tipo clinico-assistenziale sia di tipo socio-sanitario; per tali motivi, come evidenziato in letteratura, gli infermieri possono trovarsi spesso emotivamente troppo coinvolti nella presa in carico di questi neonati fino a comprommettere la qualità dell’assistenza erogata. L’obiettivo dello studio è quello di indagare il vissuto emortivo dell’infermiere e le eventuali implicazioni assistenziali riscontrate. Materiali e metodi Studio qualitativo fenomenologico con metodo di Cohen condotto attraverso interviste semi-strutturate ad alcuni infermieri afferenti alla Struttura Organizzativo Dipartimentale (S.O.D.) di Neonatologia dell’Azienda Ospedaliero Universitaria delle Marche, scelti con campionamento intenzionale. La raccolta dei dati è stata seguita dalla trascrizione verbatim delle interviste che sono state poi codificate e, una volta individuati i codici, ovvero delle parole chiave, sono stati inseriti all’interno di categorie per favorire l’analisi tematica. È stata effettuata una verifica dei temi rilevati con i partecipanti allo studio. Risultati Il campione analizzato (n=5 infermieri) si è caratterizzato per una forte eterogeneità nelle risposte, dalle quali sono emerse e sono state affrontate quattro aree tematiche principali: l’esperienza nel setting operativo che si dimostra un fondamentale strumento infermieristico per l’assistenza (I), le criticità e le difficoltà che affrontano gli infermieri durante la degenza del neonato con SAN (II), le emozioni provate dal personale infermieristico (III) e la qualità dell’assistenza erogata (IV). Analisi e discussione L’analisi delle interviste circa il vissuto emotivo degli infermieri conferma le difficoltà assistenziali con le quali questi si interfacciano durante il periodo di degenza del neonato in ospedale. Quindi, non soltanto si evidenzia la presenza di criticità clinico-assistenziali nella presa in carico di un neonato affetto da SAN, ma anche problematiche nella costruzione di una relazione terapeutica con il genitore, in particolare con la madre tossicodipendente, la quale può spesso diventare bersaglio di accuse, pregiudizi e stigmatizzazione da parte degli operatori sanitari come ampiamente discusso in letteratura. Conclusioni In conclusione, nonostante la complessità clinica e assistenziale del neonato con SAN, la qualità dell’assistenza risente positivamente del sentimento di affetto che gli infermieri provano nei suoi confronti; tuttavia, si evidenzia la necessità di una formazione specifica in merito al tema delle dipendenze patologiche nel neonato.
