L'infermiere e le cure palliative nel territorio: una revisione narrativa sull'approccio educativo-relazionale.

Introduction: Palliative care is an approach that allows for the global care of the patient and his family by an interdisciplinary team, through the development of an individualized care plan. The objective is to guarantee the quality of life and maintain the dignity of patients suffering from chronic degenerative diseases and their families at the end of life. In the home environment, which is considered the most suitable setting to allow patients to better deal with the critical and delicate phases of the disease process, the nurse together with the team is able to satisfy the physical and psychological needs of the patients. Objective: The aim of the thesis was to identify and analyze the evidence in the literature proving the current role of home nursing care in the management of patients undergoing palliative care, in therapeutic education and in the communicative relationship that the nurse should establish with the patient and the family for the purposes of effective assistance. In particular, identify the skills, the methods currently used by nurses or which should be increased, and the values they are inspired by in order to act professionally. Materials and methods: A narrative review of the literature was performed identifying the search string through the PICOM methodology. The bibliographic search for articles was carried out using keywords in the MEDLINE (Pubmed) and EBSCO (Cinhal) databases. Using the Prisma methodology, articles suitable for the review were selected. Results: Of the articles consulted, the most relevant to the research objective were selected and concern the communication and relational skills of nurses, their opinions and the preferences of patients and caregivers. Conclusions: In light of the results that emerged from the study and the analysis of the articles included in the review, it is possible to affirm that nurses play an important role in offering compassionate and humane care, respecting patients’ values and preferences. Palliative care requires critical thinking, high cognitive skills and complex nursing skills, as well as the ability to integrate humanistic and scientific aspects. In the home setting, nurses can build deep bonds with patients and caregivers, supporting them with respect and dignity until the end. Introduzione: Le Cure Palliative, sono un approccio che permettono la presa in carico globale dell’assistito e della sua famiglia da parte di un team interdisciplinare, attraverso l’elaborazione di un piano assistenziale individualizzato. L’obiettivo è quello garantire la qualità di vita e mantenere la dignità dei pazienti affetti da malattie cronico degenerative e delle loro famiglie nel periodo di fine vita. Nell’ambito domiciliare, che è considerato il setting più idoneo per permettere ai pazienti di affrontare al meglio le fasi critiche e delicate del percorso di malattia, l’infermiere insieme all’equipe riesce a soddisfare i bisogni fisici e psicologici degli assistiti. Obiettivo: Lo scopo dell’elaborato è stato individuare ed analizzare nella letteratura le evidenze provanti l’attuale ruolo dell’assistenza infermieristica domiciliare nella gestione del paziente sottoposto alle cure palliative, nell’educazione terapeutica e nella relazione comunicativa che l’infermiere dovrebbe instaurare con il paziente e la famiglia ai fini di una assistenza efficace. In particolar modo individuare le competenze, i metodi attualmente utilizzati dagli infermieri o che dovrebbero essere incrementati, e i valori ai quali si ispirano per poter agire professionalmente. Materiali e metodi: È stata eseguita una revisione narrativa della letteratura identificando la stringa di ricerca attraverso la metodologia PICOM. La ricerca bibliografica di articoli è stata effettuata attraverso delle parole chiave nelle banche dati MEDLINE (Pubmed) ed EBSCO (Cinhal). Tramite la metodologia Prisma, sono stati selezionati gli articoli idonei per la revisione. Risultati: Degli articoli consultati sono stati selezionati i più pertinenti all’obiettivo di ricerca e riguardano le capacità comunicative e relazionali degli infermieri, le loro opinioni e le preferenze dei pazienti e dei caregiver. Conclusioni: Alla luce dei risultati emersi dallo studio e dall’analisi degli articoli inclusi nella revisione, è possibile affermare che gli infermieri rivestono un ruolo importante nell’offrire cure compassionevoli e umane, rispettando i valori e le preferenze dei pazienti. Le cure palliative richiedono pensiero critico, elevate abilità cognitive e competenze infermieristiche complesse, oltre alla capacità di integrare aspetti umanistici e scientifici. Nel contesto domiciliare, gli infermieri possono instaurare legami profondi con i pazienti e i caregiver, supportandoli con rispetto e dignità fino alla fine. La formazione specifica per gli infermieri è essenziale per garantire un’assistenza di qualità basata sulle migliori evidenze.
