INTRODUCTION: Preliminary knowledge from studies on biomechanics and kinesiology tells us that the foot and the entire lower limb are continuously interconnected, however, we cannot state a priori that kinematic variations in these districts have repercussions to the point of causing a musculoskeletal pathology in other joints. OBJECTIVE: To investigate the literature to understand how plantar support and musculoskeletal pathologies of the lower limb are related. METHODS: Various searches were carried out on Pubmed. Meta-analyses and systematic reviews were taken into consideration, the studies examined had to examine plantar support, foot posture or other foot variables and at the same time a musculoskeletal pathology in the lower limb district. RESULTS: A total of 21 articles were selected, including 13 meta-analyses and 8 systematic reviews. The most significant results found that patients with gonarthrosis have a more pronated foot posture and an altered distribution of plantar pressures in the foot. Treatment with orthoses for knee problems have had some effects on the correction of biomechanical parameters but as far as pain is concerned they have proven ineffective. Furthermore, associations were observed between ankle injuries and changes in the lower limb, but also associations between lower limb injuries and certain foot postures. CONCLUSIONS: it can be stated that the topic of plantar support in correlation with musculoskeletal pathologies of the lower limb finds widespread emphasis in the literature. There have been numerous studies in favor of this link, but at the same time, there have also been some articles that have not detected this correlation. The consideration of aspects connected to the foot or plantar support allows an expansion of clinical reasoning and their inclusion also in the treatment.
INTRODUZIONE: Le conoscenze preliminari dagli studi sulla biomeccanica e dalla chinesiologia ci dicono che il piede e tutto l’arto inferiore siano continuamente interconnessi, tuttavia, non possiamo affermare a priori che variazioni cinematiche in questi distretti si ripercuotano a tal punto da causare una patologia muscoloscheletrica in altre articolazioni. OBIETTIVO: Indagare la letteratura per comprendere in che modo l’appoggio plantare e le patologie muscoloscheletriche dell’arto inferiore siano correlate. METODI: Sono state effettuate svariate ricerche su Pubmed. Sono state prese in considerazione meta-analisi e revisioni sistematiche, gli studi presi in esame dovevano esaminare appoggio plantare, postura del piede o altre variabili del piede e allo stesso tempo una patologia muscoloscheletrica nel distretto dell’arto inferiore. RISULTATI: Sono stati selezionati un totale di 21 articoli, di cui 13 metanalisi e 8 revisioni sistematiche. I risultati più significativi hanno riscontrato che i pazienti con gonartrosi hanno una postura del piede più pronata e una distribuzione delle pressioni plantari nel piede alterate. Il trattamento con le ortesi per problematiche al ginocchio hanno avuto alcuni effetti su la correzione di parametri biomeccanici ma per quanto riguarda il dolore si sono dimostrate inefficaci. Inoltre sono state osservate associazioni, tra infortuni alla caviglia e modifiche nell'arto inferiore, ma anche associazioni tra infortuni all'arto inferiore e determinate posture del piede. CONCLUSIONI: si può affermare come il tema dell’appoggio plantare in correlazione con patologie muscoloscheletriche dell’arto inferiore trovi ampio risalto nella letteratura. Gli studi a favore di questo legame sono stati numerosi, ma allo stesso tempo, sono stati riscontrati anche alcuni articoli che non hanno rilevato questa correlazione. La considerazione di aspetti connessi al piede o all’appoggio plantare permette un ampliamento del ragionamento clinico e la loro inclusione anche nel trattamento.
