Integrated home care has been evolving rapidly over the past few years , thanks in part immediately to the adoption of innovative technologies such as telehealth and artificial intelligence (AI), which have the potential to significantly improve the management of chronic conditions, particularly cardiovascular conditions. In this thesis, a review of the latest scientific publications is conducted to visualize the implementation of these technologies in the context of home nursing care, with a focus on the benefits offered by telehealth.The main objective is to demonstrate how the integration of artificial intelligence and telehealth can improve the quality of home care for patients with cardiovascular disease, reducing mortality rates and hospitalizations with a focus on improving the quality of care processes. Using this methodology, recent studies were reviewed, focusing on the clinical and operational impact in the nursing setting.The results obtained show that the use of artificial intelligence , combined with remote monitoring, improves the ability of nurses to intervene in a timely manner, reducing the risk of serious complications and increasing therapeutic adherence of patients with a more than significant effect in care outcomes. Studies on telehealth have also shown a reduction in healthcare costs by optimizing the use of hospital resources and ensuring continuous and personalized monitoring.In the discussion, it is emphasized that despite the benefits, there are still challenges to be addressed, including training nurses to expand their skills and patient acceptance of these technologies, especially for today's elderly population. However, the integration of AI and telemonitoring into the health care system is an important step toward the evolution of home care, with the nurse assuming a central role as a technological and clinical facilitator.The research encapsulated numerous studies that lead to the conclusion to invest in training and technological infrastructure to make the most of these innovations,demonstrating the crucial role that the nurse will play in the future of integrated home care through the combined use of AI and telehealth. the future of integrated home care and home hospitalization represent the new care delivery pathway of the health care system, thereby addressing the problem of inpatient costs and insufficient beds.
L'assistenza domiciliare integrata si sta evolvendo rapidamente nel corso degli ultimi anni , anche grazie subito all’adozione di tecnologie innovative come la teleassistenza e l’intelligenza artificiale (IA), che hanno il potenziale di migliorare significativamente la gestione delle patologie croniche, in particolare quelle cardiovascolari. Nella tesi viene effettuata una revisione delle ultime pubblicazioni scientifiche per visualizzare l’implementazione di queste tecnologie nel contesto dell’assistenza infermieristica domiciliare, con particolare attenzione ai benefici offerti dalla teleassistenza.L'obiettivo principale è dimostrare come l’integrazione dell’intelligenza artificiale e teleassistenza possano migliorare la qualità dell'assistenza domiciliare ai pazienti con patologie cardiovascolari, riducendo i tassi di mortalità e i ricoveri ospedalieri con particolare attenzione nel migliorare la qualità dei processi assistenziali. Utilizzando questa metodologia, sono stati esaminati studi recenti, focalizzandosi sull'impatto clinico e operativo nel contesto infermieristico.I risultati ottenuti dimostrano che l'utilizzo dell'intelligenza artificiale , combinata con il monitoraggio a distanza, migliora la capacità degli infermieri di intervenire tempestivamente, riducendo il rischio di complicanze gravi e aumentando l'aderenza terapeutica dei pazienti con un effetto più che significativo negli outcomes assistenziali. Gli studi sulla teleassistenza hanno anche evidenziato una riduzione dei costi sanitari, ottimizzando l'uso delle risorse ospedaliere e garantendo un monitoraggio continuo e personalizzato.Nella discussione, viene sottolineato come, nonostante i benefici, vi siano ancora sfide da affrontare, tra cui la formazione degli infermieri in modo da ampliare le loro competenze e l'accettazione di queste tecnologie da parte dei pazienti, specialmente per la popolazione anziana attuale. Tuttavia, l’integrazione di IA e telemonitoraggio nel sistema sanitario rappresenta un passo importante verso l'evoluzione dell’assistenza domiciliare, con l’infermiere che assume un ruolo centrale come mediatore tecnologico e clinico.la ricerca racchiuso numerosi studi che portano alla conclusione di investire in formazione e infrastrutture tecnologiche per sfruttare al meglio queste innovazioni,dimostrando il ruolo cruciale che l’infermiere svolgerà nel futuro dell’assistenza domiciliare integrata, grazie all’uso combinato di IA e teleassistenza. il futuro dell’assistenza domiciliare integrata e l’ospedalizzazione domiciliare rappresentano la nuova via di erogazione di assistenza del sistema sanitario, andando così a rispondere al problema dei costi di degenza e insufficienza di posti letto.
