Peritoneal dialysis (PD) is a treatment modality for chronic kidney disease. In the context of peritoneal dialysis, the nurse is a figure of particular importance in the care management of patients, he is in fact involved in various phases of the patient care process such as training, follow-up and any educational interventions, where deemed necessary. In various studies it has been demonstrated that the educational process is of fundamental importance in home treatment, and the nurse uses various tools such as training, home visits and re-training to implement this process. Home visits in this context take on a strong relevance as they allow the patient's educational activity to be continued consistently throughout the therapy, and problems or critical issues relating to compliance with the procedures specific to the technique and adherence to prescriptions to be detected, through forms of evaluation. It has also been demonstrated from the data present in the literature that a good home visit program reduces the rate of infections in general, peritonitis, drop-outs and hospitalizations; on the other hand, you increase the survival rate and overall adherence to therapy.
La dialisi peritoneale (DP) è una modalità di trattamento per l’insufficienza renale cronica. Nell’ambito della dialisi peritoneale, l’infermiere è una figura di particolare rilevanza nella gestione assistenziale dei pazienti, esso è infatti coinvolto in varie fasi del percorso di cura del paziente quali l’addestramento, follow-up e gli eventuali interventi educazionali, laddove ritenuti necessari. In diversi studi si è dimostrato come il processo educativo, sia di fondamentale importanza nel trattamento domiciliare, e l’infermiere per l’attuazione di tale processo si avvale di diversi strumenti come il training, le visite domiciliari e il re-training. Le visite domiciliari in questo contesto assumono una forte rilevanza in quanto permettono di proseguire l’attività educazionale del paziente con costanza durante tutta la terapia, rilevare problematiche o criticità relative alla compliance nelle procedure proprie della tecnica e nell’aderenza delle prescrizioni, attraverso delle schede di valutazione. Si è inoltre dimostrato dai dati presenti in letteratura che un buon programma di visite domiciliari riduca il tasso di infezioni in generale, di peritoniti, drop-out e di ospedalizzazioni; d’altra parte, aumenti il tasso di sopravvivenza e in generale l’aderenza alla terapia.
