The objective of the thesis is to identify the best assessment and nursing intervention strategies to be implemented on pediatric patients affected by intestinal volvulus in all the various care moments, from the acute phase to the perioperative course, from hospitalization in the ward to the return home, in order to promptly recognize the signs and symptoms and prevent complications. The method used is focused on the analysis of the hospital path adopted by the healthcare team that took charge of the patient affected by volvulus of the sigmoid colon and the therapeutic strategies adopted at home by the caregiver. We also wanted to compare all the strategies used with the available scientific literature in order to evaluate their actual appropriateness. To find the material on the clinical case under analysis, I made use of an interview that I personally administered to the patient and her family members, going to their home. The processing of sensitive data and the consultation of clinical reports were possible with the consent of the parents. The complex management of the clinical case examined fortunately ended with the achievement of an optimal state of health in which the patient currently finds herself, despite the difficulties of differential diagnosis encountered also related to the season of the year in which the case presented itself, because compatible with a high incidence of cases of gastroenteritis. During the operation, the presence of a colon longer than normal (Dolichocolon) was also found, which was the cause of the volvulus but which also avoided the patient from creating a stoma, given the need for resection of 40 cm of necrotized sigma colon.
Obiettivo della tesi è quello di individuare le migliori strategie di accertamento e di intervento infermieristico da attuare sui pazienti pediatrici affetti da volvolo intestinale in tutti i vari momenti assistenziali, dalla fase acuta al decorso perioperatorio, dalla degenza in reparto fino al rientro al domicilio, al fine di riconoscerne tempestivamente i segni e i sintomi e prevenirne le complicanze. Il metodo utilizzato è incentrato sull’analisi del percorso ospedaliero adottato dall’ equipe sanitaria che ha preso in carico la paziente affetta da volvolo del colon sigma e delle strategie terapeutiche adottate a domicilio dal caregiver. Si è voluto anche confrontare tutte le strategie utilizzate con la letteratura scientifica disponibile per poterne valutare l’effettiva appropriatezza. Per il reperimento del materiale sul caso clinico preso in analisi, mi sono avvalsa dell’uso di un’intervista che ho personalmente somministrato alla paziente e ai familiari, recandomi presso il loro domicilio. Il trattamento dei dati sensibili e le consultazioni dei referti clinici sono stati possibili previo consenso dei genitori. La complessa gestione del caso clinico preso in esame si è fortunatamente conclusa con il raggiungimento di uno stato di salute ottimale in cui attualmente si trova la paziente, nonostante le difficoltà di diagnosi differenziale incontrate legate anche alla stagione dell’anno in cui il caso si è presentato, perché compatibile con una alta incidenza di casi di gastroenterite. In sede di intervento si è inoltre riscontrata la presenza di un colon più lungo del normale (Dolicocolon) che, è stata sì la causa del volvolo ma che ha anche evitato alla paziente il confezionamento di una stomia, data la necessità di resezione di 40 cm di colon sigma necrotizzato.
