Introduction: the management of a patient with traumatic hemorrhage of the limbs in the extra-hospital context represents a great challenge. A valid alternative to traditional dressings is the use of the Tourniquet, a device of military origin through which it is possible to stop the hemorrhage until transport to the nearest Trauma Center. Objective: the primary objective of the study is to investigate the knowledge of the “Stop the Bleed” protocol by the nursing staff in service in the Territorial Emergency Unit – 118 of the AST of Ascoli Piceno. The secondary objective is to identify the level of training on the use of the Tourniquet. Materials and methods: a cross-sectional, monocentric observational study was conducted that does not include a diagnostic-therapeutic approach. The data were collected by consulting scientific databases and by administering the validated questionnaire (C. Ramacciani, Scenario ANIARTI, 2023) to the nurses of the 118 of the Hospital of Ascoli Piceno, San Benedetto del Tronto and the Potes of Offida. Results: from the statistical analysis, a maximum adhesion to the study was found with a Response Rate of 97.3%. It is clear that 41.6% are aware of the “Stop the Bleed” Protocol but unfortunately 63.9% of the interviewees are not aware of this protocol at company level. 91.7% are aware of the Combat Tourniquet device and 50% of all nurses are inclined to prepare an Emergency Kit to stop bleeding as long as there is adequate training (83.3%). Discussion: the research carried out highlights of the main problem; a lack of training regarding the “Stop the Bleed” protocol both during university and work careers. On the other hand, the data regarding knowledge of the Tourniquet device are encouraging. Through the consultation of the studies, it is clear that the use of the tourniquet seems to be associated with effective control of hemorrhage and that it can contribute to the reduction of deaths due to hemorrhage. Conclusion: thanks to this study, it is highlighted that it is necessary to increase shared programs on the management of external hemorrhages of the limbs through the use of adequate devices to obtain immediate control of the hemorrhage without further complications.
Introduzione: la gestione di un paziente con emorragia traumatica degli arti nel contesto extra – ospedaliero, rappresenta una grande sfida. Una valida alternativa alle medicazioni tradizionali, è l’utilizzo del Tourniquet, un dispositivo di origine militare attraverso il quale è possibile arrestare l’emorragia fino al trasporto presso il Trauma Center più vicino. Obiettivo: l’obiettivo primario dello studio, è quello di indagare la conoscenza del protocollo “Stop the Bleed” da parte del personale infermieristico in servizio nell’U.O. di Emergenza Territoriale – 118 dell’AST di Ascoli Piceno. L’obiettivo secondario invece, è quello di individuare il livello di formazione sull’utilizzo del Tourniquet. Materiali e metodi: è stato condotto uno studio osservazionale trasversale, monocentrico che non prevede un approccio diagnostico – terapeutico. I dati sono stati raccolti attraverso la consultazione di banche dati scientifiche e tramite la somministrazione del questionario validato (C. Ramacciani, Scenario ANIARTI, 2023) agli infermieri del 118 dell’Ospedale di Ascoli Piceno, San Benedetto del Tronto e della Potes di Offida. Risultati: dall’analisi statistica, si riscontra una massima adesione allo studio con un Response Rate del 97,3%. Si evince che il 41,6% è a conoscenza del Protocollo “Stop the Bleed” ma purtroppo il 63,9% degli intervistati non è a conoscenza di tale protocollo a livello aziendale. Il 91,7% è a conoscenza del presidio Combat Tourniquet e un 50% di tutti gli infermieri, sono propensi alla predisposizione di un Kit di Pronto Intervento per l’arresto delle emorragie purché ci sia un’adeguata formazione (83,3%). Discussione: la ricerca svolta mette in luce il problema principale; una carenza di formazione riguardo il protocollo “Stop the Bleed” sia durante la carriera universitaria che quella lavorativa. Incoraggianti invece, sono i dati riguardanti la conoscenza del presidio Tourniquet. Attraverso la consultazione degli studi, si evince che l’impiego del tourniquet sembri essere associato ad un efficace controllo dell’emorragia e che possa contribuire alla diminuzione dei decessi per emorragia. Conclusione: grazie a questo studio, si evidenzia che occorre incrementare i programmi condivisi sulla gestione delle emorragie esterne degli arti attraverso l’utilizzo di adeguati presidi per ottenere un controllo immediato dell’emorragia senza ulteriori complicanze.
