Abstract INTRODUCTION: The World Health Organization (WHO) defines child maltreatment and abuse as “All forms of physical and emotional maltreatment, sexual abuse, neglect and exploitation that result in actual or potential harm to the child’s health, development or dignity”. (WHO, 2021) The diagnostic activity to which patients with child maltreatment are subjected is extremely complex for multiple reasons, “the victim may be non-verbal or too scared or seriously injured”. (Dulce Gonzalez 1, 2024) Furthermore, very often the phenomenon is largely minimized or even denied both due to difficult cultural contexts, but also due to its occurrence in a domestic environment, which is why it very often remains unexpressed and invisible. This therefore determines a significant underestimation of the phenomenon itself. The primary approach between the child victim of abuse and the health system is represented by the Emergency Department and the professionals who compose it. It is during the triage process in fact that the patient's data and medical history are collected, which can lead to suspecting violence. It is therefore necessary that the professional is adequately trained to recognize any signs and symptoms and to correctly apply the Procedures and Protocols in force within his/her own Hospital Company of competence. OBJECTIVES: The objective of this paper is based on the detection and analysis of the phenomenon of child abuse with a view to access to the Emergency Departments of the AST of Ascoli Piceno, which includes the Hospital Establishments "C. & G. Mazzoni" of Ascoli Piceno and the "Madonna del Soccorso" of San Benedetto del Tronto. The suspicion of child abuse by the healthcare personnel therefore allows the activation of the appropriate paths (Pink Code and Red Code), which guarantee the minor the most appropriate treatment in relation to the type of abuse suffered. All this is possible through the Procedures and Protocols currently in force at the AST.
Abstract INTRODUZIONE: L’ Organizzazione Mondiale della Sanità (OMS) definisce il maltrattamento e l’abuso infantile come “Tutte le forme di maltrattamento fisico ed emotivo, abuso sessuale, negligenza e sfruttamento che provocano danni reali o potenziali alla salute, allo sviluppo o alla dignità del bambino”. (WHO, 2021) L’attività diagnostica cui sono sottoposti i pazienti con maltrattamento infantile è estremamente complessa per molteplici cause, “la vittima potrebbe essere non verbale o troppo spaventata o gravemente ferita”. (Dulce Gonzalez 1, 2024) Inoltre, molto spesso il fenomeno viene largamente minimizzato o addirittura negato sia per contesti culturali difficili, ma anche per il suo verificarsi in ambiente domestico, ragion per cui rimane molto spesso inespresso ed invisibile. Ciò determina dunque una rilevante sottostima del fenomeno in sé. L’approccio primario tra il minore vittima di abuso ed il sistema sanitario è rappresentato dal Pronto Soccorso e dai professionisti che lo compongono. È durante il percorso di triage infatti, che vengono raccolti i dati e l’anamnesi del paziente, che possono spingere a sospettare la violenza. È necessario perciò che il professionista sia adeguatamente formato a riconoscere eventuali segni e sintomi e ad applicare correttamente le Procedure ed i Protocolli in vigore all’interno della propria Azienda Ospedaliera di competenza. OBIETTIVI: L’obiettivo di questo elaborato si basa sulla rilevazione ed analisi del fenomeno del maltrattamento minorile in ottica di accesso presso i Pronto Soccorso dell’AST di Ascoli Piceno, la quale comprende gli Stabilimenti Ospedalieri “C.&G. Mazzoni” di Ascoli Piceno ed il “Madonna del Soccorso” di San Benedetto del Tronto. Il sospetto di maltrattamento minorile da parte del personale sanitario permette dunque l’attivazione degli appositi percorsi (Codice Rosa e Codice Rosso), i quali garantiscono al minore il trattamento più adeguato in relazione alla tipologia di abuso subito. Tutto ciò è possibile per mezzo delle Procedure e dei Protocolli attualmente in vigore presso l’AST.
