Purpose: To evaluate the correlations between anatomical and functional changes in idiopathic macular hole (IMH) surgery in long-term follow-up. Methods: This is a prospective, interventional single centre case series. The final sample is formed by 14 eyes of 14 patients who had undergone IMH surgery in our institution between 2005 and 2009 and were still followed up in 2019. Reported data were pre- and post-operative best-corrected visual acuity (BCVA), retinal sensitivity and fixation stability values on MP-1 and structural macular features on spectral domain optical coherence tomography. Optical coherence tomography angiography (OCTA) was utilized to assess microvascular characteristics at the final visit. Only patients with a successful MH closure were enrolled, excluding eyes affected by other relevant pathologies. Results: Mean BCVA improvement was significant after surgery (t test, p value < 0.001) and presented a slight, not statistically significant raise, between the post-operative and long-term follow-up. Differently, mean retinal sensibility (dB) showed a highly statistically significant difference between pre-operative and post-operative (t test, p value = .002) and post-operative and last follow-up (p value < 0.001). In the long-term follow-up, subjects having integrity of the inner segment/outer segment (IS/OS) layer showed no statistically significant difference in BCVA compared with subjects with IS/OS discontinuity (t test, p value = 0.72). OCTA parameters of the operated eye showed no statistical significance compared with the fellow eye. Conclusions: In successfully closed MHs, retinal sensibility measured by microperimetry significatively increases after a long follow-up period even when BCVA remains stable or raises slightly. Vessel density organization tends to be quantitatively similar to fellow eye several years after surgery.
Scopo dello studio: Valutare le correlazioni tra i cambiamenti anatomici e funzionali nella chirurgia del foro maculare idiopatico (IMH) nel follow-up a lungo termine. Metodi: Questa è una serie prospettica, interventistica di casi a singolo centro. Il campione finale è formato da 14 occhi di 14 pazienti che erano stati sottoposti a chirurgia IMH nel nostro istituto tra il 2005 e il 2009 e sono stati ancora seguiti nel 2019. I dati riportati erano acuità visiva meglio corretta pre e post-operatoria (BCVA), valori di sensibilità retinica e stabilità di fissazione su MP-1 e caratteristiche maculari strutturali su tomografia a coerenza ottica a dominio spettrale. L'angiografia con tomografia a coerenza ottica (OCTA) è stata utilizzata per valutare le caratteristiche microvascolari alla visita finale. Sono stati arruolati solo pazienti con una chiusura con successo dell'MH, esclusi gli occhi affetti da altre patologie rilevanti. Risultati: Il miglioramento medio della BCVA è stato significativo dopo l'intervento chirurgico (t test, p value <0,001) e ha presentato un leggero, non statisticamente significativo aumento, tra follow-up post-operatorio e follow-up a lungo termine. Diversamente, la sensibilità media della retina (dB) ha mostrato una differenza molto statisticamente significativa tra preoperatorio e post-operatorio (t test, p value = .002) e post-operatorio e ultimo follow-up (p value <0,001). Nel follow-up a lungo termine, i soggetti con integrità del livello di segmento interno / segmento esterno (IS / OS) non hanno mostrato differenza statisticamente significativa nella BCVA rispetto ai soggetti con discontinuità IS / OS (t test, p value = 0,72). I parametri OCTA dell'occhio operato non hanno mostrato alcun significato statistico rispetto all’altro occhio. Conclusioni: In MH chiusi con successo, la sensibilità della retina misurata dalla microperimetria aumenta significativamente dopo un lungo periodo di follow-up anche quando la BCVA rimane stabile o aumenta leggermente. L'organizzazione della densità vascolare tende ad essere quantitativamente simile a quella dell'altro occhio diversi anni dopo l'intervento chirurgico.
