Introduction Borderline Personality Disorder(BPD) is a complex psychiatric disorder characterized by emotional instability, relationship difficulties and impulsivity, often associated with self-harm and suicide attempts. Its management requires specialized clinical skills and a targeted care approach. Objective The study aims to examine the caregiving difficulties in managing a caregiver with BDP. Material and methods The study considers a case report of a 17-year-old girl with BDP; the history was reconstructed by semi-structured interview, analyzing her diagnostic-therapeutic-care pathway. The analysis was conducted using a descriptive and qualitative approach, with a focus on the therapeutic relationship and care setting. Results This case study highlighted the main care issues in the caretaking of BDP, among which late diagnosis, difficulties in building a stable therapeutic relationship, and the need to integrate hospital and territorial facilities to ensure continuity of care emerge from the interview; the need to build specific care settings for minors also emerges from the interview. Discussion and Conclusions The case analysis highlights the central role of the nurse in addressing the main critical issues that emerged such as the difficulty in building a stable therapeutic relationship. Structured nursing care, based on specialized expertise and effective integration between hospital and territory, is essential to ensure adequate support, improve treatment adherence and reduce the risk of relapse, and ultimately achieve positive outcomes. Introduzione Il Disturbo Borderline di Personalità (BPD) è una patologia psichiatrica complessa, caratterizzata da instabilità emotiva, difficoltà relazionali e impulsività, spesso associata ad autolesionismo e tentativi di suicidio. La sua gestione richiede competenze cliniche specializzate e un approccio assistenziale mirato. Obiettivo Lo studio ha l’obiettivo di esaminare le criticità assistenziali nella gestione di un assistito con BPD. Materiale e metodi Lo studio prende in considerazione un case report di una ragazza di 17 anni affetta da BDP, l’anamnesi è stata ricostruita mediante intervista semi-strutturata, analizzando il suo percorso diagnostico-terapeutico-assistenziale. L'analisi è stata condotta utilizzando un approccio descrittivo e qualitativo, con particolare attenzione alla relazione terapeutica e al setting assistenziale. Risultati Lo studio di questo caso ha evidenziato le principali problematiche assistenziali nella presa in carico del BDP, tra cui emerge la diagnosi tardiva, le difficoltà nel costruire una relazione terapeutica stabile e la necessità di integrare strutture ospedaliere con quelle territoriale per garantire la continuità assistenziale; dall’intervista emerge inoltre la necessità di costruire setting assistenziali specifici per i minori. Discussione e Conclusioni Dall’analisi del caso si evidenzia il ruolo centrale dell’infermiere nell’affrontare le principali criticità emerse come la difficoltà nel costruire una relazione terapeutica stabile. Un’assistenza infermieristica strutturata, basata su competenze specialistiche e un’integrazione efficace tra ospedale e territorio, è essenziale per garantire un supporto adeguato, migliorare l’aderenza al trattamento e ridurre il rischio di ricadute, per ottenere infine degli esiti positivi.
