Abstract Introduction and Objectives: The Italian healthcare system is evolving towards a more integrated territorial model, driven by the aging population, the rise in chronic diseases, and the experience of the pandemic. The analysis focuses on the healthcare district of Ascoli Piceno, evaluating the number of patient admissions and management in the OdC (Community Hospital) and COT (Territorial Operational Center) facilities, with a specific analysis of the role of the Community Nurse. Materials and Methods: The study adopts a structured methodological approach, combining two main research strategies: an in-depth document review and a quantitative data analysis. Numerical data related to admissions and patients were collected through direct meetings with the coordinators of the Territorial Operational Center (COT), the Community Hospital (OdC), and their respective management software. Results: The analysis highlights how the implementation of new healthcare models in the AST of Ascoli Piceno aligns with the targets set by the PNRR, contributing to the reduction of inappropriate hospitalizations and improving the daily quality of life for patients. Data from the healthcare settings during the second half of 2024, from June to December, are taken into consideration. Analysis: The analysis provides an overview of the territorial management of the Ascoli Piceno healthcare district, which is part of the AST Ascoli Piceno, in light of the reorganization dictated by the PNRR. Discussion: The data on the territorial healthcare services in Ascoli are compared to the targets set by the PNRR. Abstract Introduzione e Obiettivi: Il sistema sanitario italiano sta evolvendo verso un modello territoriale più integrato catalizzato dall'invecchiamento della popolazione, dall'aumento delle malattie croniche e dall'esperienza pandemica. L'analisi si concentra sul distretto sanitario di Ascoli Piceno, valutando il numero di accessi e gestione dei pazienti nelle strutture OdC e COT, si effettua un'analisi sulla figura dell’Infermiere di comunità. Materiali e Metodi: Lo studio adotta un approccio metodologico articolato, combinando due principali strategie di ricerca: una revisione documentale approfondita e un'analisi quantitativa dei dati. I dati numerici relativi agli accessi e agli assistiti sono stati raccolti attraverso incontri diretti con i coordinatori della Centrale Operativa Territoriale (COT), dell'Ospedale di Comunità (OdC) e dai loro software gestionali di riferimento. Risultati: L’analisi evidenzia come l’implementazione dei nuovi modelli assistenziali nell’AST di Ascoli Piceno sia in linea con i target fissati dal PNRR, contribuendo a ridurre le ospedalizzazioni inappropriate e a migliorare la qualità della vita quotidiana dei pazienti. Vengono presi in considerazione i dati dei setting assistenziali nel periodo del secondo semestre del 2024, da giugno a dicembre. Analisi: L’analisi offre una panoramica generale della gestione territoriale del distretto sanitario di Ascoli Piceno, facente parte dell’AST Ascoli Piceno, alla luce della riorganizzazione dettata dal PNRR. Discussione: Vengono messi a confronto i dati sull’assistenza territoriale ascolana rispetto ai target dettati dal PNRR.
