Background. Ankyloglossia, or tongue-tie, is a congenital condition that may restrict tongue mobility, with potential impact on sucking, swallowing, breathing, speech, and craniofacial development. Reported prevalence ranges from 4% to 10%, reflecting the lack of standardized diagnostic criteria and the heterogeneity of clinical management. Recently, increasing attention has been given to combined treatment, integrating surgical release with Orofacial Myofunctional Therapy (OMT), aimed at reducing relapses and improving functional outcomes. However, available evidence remains limited and inconsistent. Materials and Methods. A systematic review was conducted according to PRISMA 2020 guidelines, searching PubMed, Scopus, and Cochrane Library for studies published in the last 10 years, including subjects older than 4 years treated with surgery combined with OMT. Case reports, case series, and uncontrolled studies were excluded. Methodological quality was assessed with RoB-2 and ROBINS-I. In addition, a project proposal was developed, consisting of a regional survey addressed to speech therapists and medical specialists in the Marche Region, together with an interdisciplinary training event to be held at a regional conference. Results. Of 107 initial records, only 3 met eligibility criteria (2 RCTs and 1 prospective controlled study). Findings suggest that combined surgery and OMT improve tongue mobility, mouth opening, and some oral functions compared with single interventions, while speech outcomes remain inconsistent. Major limitations include small sample sizes, non-standardized protocols, heterogeneous assessment tools, and short follow-up. Conclusions. The integration of surgery and Orofacial Myofunctional Therapy represents a clinically advantageous and promising approach for the management of ankyloglossia. Despite methodological limitations, current evidence reinforces the need for multidisciplinary therapeutic pathways capable of translating observed benefits into standardized and shared clinical practices. Further studies with larger samples, homogeneous protocols, and longer follow-up are required. Within this framework, the proposed regional survey and training initiative offer a useful model to promote integrated, evidence-based care pathways and support the perspective of including OMT within the Italian Essential Levels of Care.
Background. L’anchiloglossia, o frenulo linguale alterato, è una condizione congenita che può limitare la mobilità della lingua interferendo con le funzioni orali. La prevalenza riportata in letteratura è variabile (circa 4–10%), riflettendo la mancanza di criteri diagnostici condivisi e la conseguente eterogeneità delle pratiche cliniche. Negli ultimi anni è aumentato l’interesse per l’approccio integrato che associa l’intervento chirurgico alla Terapia Miofunzionale Orofacciale (TMF), con l’obiettivo di ridurre recidive e ottimizzare gli esiti funzionali. Le evidenze scientifiche disponibili, tuttavia, sono ancora limitate ed eterogenee. Materiali e metodi. È stata condotta una revisione sistematica secondo le linee guida PRISMA 2020, ricercando su PubMed, Scopus e Cochrane Library articoli pubblicati negli ultimi 10 anni, relativi a pazienti >4 anni sottoposti a chirurgia combinata a TMF. Sono stati esclusi case report, case series e studi senza gruppo di confronto. La qualità metodologica è stata valutata con RoB-2 e ROBINS-I. Parallelamente, è stata sviluppata una proposta progettuale di indagine conoscitiva tramite questionari rivolti a logopedisti e medici specialisti della Regione Marche e un’azione divulgativo-formativa interdisciplinare da realizzarsi in occasione di un convegno regionale. Risultati. Dai 107 articoli iniziali, solo 3 hanno soddisfatto i criteri di inclusione (2 RCT e 1 studio prospettico controllato). I dati suggeriscono che la combinazione chirurgia e TMF favorisca il recupero della mobilità linguale, dell’apertura orale e di alcune funzioni orali rispetto agli interventi isolati. Gli outcome sul linguaggio restano invece inconsistenti. I principali limiti riguardano ridotte dimensioni campionarie, protocolli non standardizzati, strumenti di valutazione eterogenei e follow-up di breve durata. Conclusioni. L’integrazione tra chirurgia e Terapia Miofunzionale Orofacciale si configura come un approccio clinicamente vantaggioso e promettente nella gestione dell’anchiloglossia. Pur a fronte di limiti metodologici, le evidenze disponibili rafforzano l’esigenza di percorsi terapeutici multidisciplinari in grado di tradurre i benefici osservati in pratiche cliniche standardizzate e condivise. Restano necessari studi futuri con campioni più ampi, protocolli omogenei e follow-up prolungati. In questo quadro, la proposta di indagine regionale e di azione formativa rappresenta un modello utile a promuovere percorsi integrati e basati sull’evidenza, sostenendo la prospettiva di una futura inclusione della TMF nei Livelli Essenziali di Assistenza.
