Hereditary spastic paraparesis (HSP) is a hereditary neurodegenerative disease characterized by spasticity and progressive weakness of the lower limbs. The aim of this study was to develop and apply a hydrokinesitherapy protocol aimed at managing spasticity and improving motor function in a patient with HSP. The treatment, consisting of five weeks of sessions in heated water, integrated mobilization exercises, muscle strengthening, postural control, and gait training. Clinical evaluation, performed using standardized functional scales, showed a general improvement in spasticity, balance, muscle strength, and quality of life. The results confirm the effectiveness of hydrokinesitherapy as a complementary rehabilitation approach in the management of spastic forms, supporting its use in therapeutic programs dedicated to pure spastic paraparesis. La paraparesi spastica pura (HSP) è una malattia neurodegenerativa ereditaria caratterizzata da spasticità e debolezza progressiva degli arti inferiori. Questo lavoro ha avuto l’obiettivo di sviluppare e applicare un protocollo di idrochinesiterapia mirato alla gestione della spasticità e al miglioramento della funzionalità motoria in una paziente affetta da HSP. Il trattamento, articolato in cinque settimane di sedute in acqua riscaldata, ha integrato esercizi di mobilizzazione, rinforzo muscolare, controllo posturale e training del cammino. La valutazione clinica, effettuata tramite scale funzionali standardizzate, ha evidenziato un miglioramento generale della spasticità, dell’equilibrio, della forza muscolare e della qualità della vita. I risultati ottenuti confermano l’efficacia dell’idrochinesiterapia come approccio riabilitativo complementare nella gestione delle forme spastiche, supportando il suo impiego nei programmi terapeutici dedicati alla paraparesi spastica pura.
