Introduction: The smile, a universal and immediate gesture, represents a powerful tool for communication and relationships. In nursing practice, it takes on a therapeutic value, capable of transmitting empathy, reducing anxiety and improving the care experience. Objective: To analyze and explore the role of the smile in nursing practice, highlighting its significance as a communicative, relational and emotional tool, and its contribution to the humanization of the care trajectory. Materials and Methods: The work is configured as a phenomenological study, enriched by a literature review and direct observations carried out during clinical placements, recorded in an observation diary. The approach adopted was both objective (etic), to assess frequency and context of the smile, and subjective (emic), to value the lived meaning. Discussion: The data show how smiling fosters trust and closeness, with evident effects in pediatrics, inpatient wards and home care. Even in adults and the elderly, smiling retains a central role, as it helps reduce stress, a sense of loneliness and promote greater relational well-being. For nurses it represents a precious personal resource, capable of counteracting burnout and strengthening professional motivation. Conclusions: The smile is confirmed as a simple but essential gesture, which combines technical competence and humanity. It is not just a relational act, but a real care tool, capable of improving the quality of care and the dignity of the person. Introduzione: Il sorriso, gesto universale e immediato, rappresenta un potente strumento di comunicazione e relazione. Nella pratica infermieristica assume un valore terapeutico, capace di trasmettere empatia, ridurre l’ansia e migliorare l’esperienza di cura. Obiettivo: Analizzare ed esplorare il ruolo del sorriso nella pratica infermieristica, evidenziandone il significato come strumento comunicativo, relazionale ed emotivo, e il suo contributo all’umanizzazione del percorso di cura. Materiali e Metodi: Il lavoro si configura come uno studio fenomenologico, arricchito da una revisione della letteratura e da osservazioni dirette svolte durante i tirocini clinici, registrate in un diario dell’osservazione. L’approccio adottato è stato sia oggettivo (etic), per valutare frequenza e contesto del sorriso, sia soggettivo (emic), per valorizzarne il significato vissuto. Discussione: I dati mostrano come il sorriso favorisca fiducia e vicinanza, con effetti evidenti in pediatria, nei reparti di degenza e nell’assistenza domiciliare. Anche negli adulti e negli anziani il sorriso conserva un ruolo centrale, poiché aiuta a ridurre lo stress, il senso di solitudine e a favorire un maggior benessere relazionale. Per gli infermieri rappresenta una preziosa risorsa personale, in grado di contrastare il burnout e di rafforzare la motivazione professionale. Conclusioni: Il sorriso si conferma come un gesto semplice ma essenziale, che unisce competenza tecnica e umanità. Non è solo un atto relazionale, ma un vero e proprio strumento di cura, capace di migliorare la qualità dell’assistenza e la dignità della persona.
