Introduction. Workplace violence is a growing phenomenon that affects various sectors, including healthcare, with negative consequences for workers. This issue is particularly evident in emergency departments and psychiatric services, where violence may come from patients, visitors, or family members. Literature shows positive correlations between numerous training programs focused on de-escalation techniques and increased staff confidence in preventing, recognizing, and managing aggressive behavior. The aim of this review is to evaluate how much a training event can improve healthcare workers’ confidence in managing violent episodes. Materials and Methods. An open search was first carried out using the main keywords via Google Scholar. Subsequently, using MeSH terms, the MEDLINE database was consulted through PubMed to collect relevant data on the main training interventions capable of improving healthcare workers’ confidence in managing aggression and violence. Results/Analysis. Seven studies were included in this literature review: two observational studies, four quasi-experimental studies, and one randomized controlled trial. All the studies analyzed the results of the Confidence in Coping with Patient Aggression (CCPAI) questionnaire before and after a training intervention, highlighting an increase in the confidence level of healthcare workers in managing aggressive behavior after attending specific training courses on de-escalation techniques. The review also showed that the differences in the average CCPAI scores before and after the training were more significant when the training included a design alternating theoretical sessions with practical ones. In particular, training that involved role-playing scenarios to simulate aggression and practice de-escalation techniques proved more effective than training consisting solely of theoretical content without practical application. Discussion/Conclusion. Specific training interventions aimed at managing aggression and violence against healthcare workers, especially those designed with a theoretical and practical learning approach, significantly increase healthcare personnel’s confidence in dealing with aggressive behavior across various clinical settings. This contributes to a reduction in issues such as burnout, turnover, or even career abandonment. Given the growing scale of workplace violence in the healthcare sector—often underreported or not reported at all—some evidence suggests that de-escalation training should be extended not only to healthcare professionals but also to students enrolled in healthcare degree programs, such as nursing. Introduzione. La violenza sul posto di lavoro è un fenomeno in costante aumento che colpisce vari settori, tra cui quello sanitario con effetti negativi sugli operatori. Questo fenomeno si manifesta in particolare nelle aree di emergenza e nei servizi psichiatrici da parte di pazienti, visitatori o familiari. In letteratura emergono correlazioni positive tra numerosi programmi formativi sull’utilizzo delle tecniche di de-escalation con lo scopo di aumentare la fiducia del personale per prevenire, riconoscere e gestire l’agito aggressivo. L’obiettivo di questa revisione è quello di valutare quanto un evento formativo sia in grado di migliorare la fiducia degli operatori nel gestire gli episodi di violenza. Materiali e metodi. È stata effettuata una ricerca libera con le principali parole chiave tramite GOOGLE SCHOLAR, successivamente con l’utilizzo dei termini MeSH, è stata consultata la banca dati MEDLINE tramite PubMed, al fine di raccogliere dati pertinenti sui principali interventi formativi in grado di migliorare la fiducia degli operatori sanitari nella gestione dell’aggressività e della violenza. Risultati/Analisi. Sette studi sono stati inclusi in questa revisione della letteratura, dei quali due studi osservazionali, quattro studi quasi sperimentali e uno studio randomizzato controllato. Tutti gli studi inclusi nella revisione hanno analizzato i risultati del questionario Confidence in Coping with Patient Aggression (CCPAI), prima e dopo un intervento formativo, evidenziando un aumento del livello di fiducia nel gestire comportamenti aggressivi da parte degli operatori sanitari che avevano partecipato ad un corso di formazione specifico sulle tecniche di de-escalation e dell’aggressività. Dalla revisione emerge inoltre che le differenze dei punteggi medi del questionario CCPAI prima e dopo l’intervento formativo, sono risultate maggiormente significative quando la formazione prevedeva una progettazione alternando una parte teorica con una parte pratica, in cui i partecipanti potessero simulare situazioni di aggressività e violenza tramite role-playing e attuare le tecniche di de-escalation apprese; rispetto ad una formazione costituita soltanto da una parte teorica senza un’applicazione pratica delle tecniche di de-escalation. Discussione/Conclusione. Gli interventi formativi specifici, finalizzati alla gestione dell’aggressività e violenza contro gli operatori sanitari, progettati con una logica di apprendimento teorico e applicazione pratica delle conoscenze apprese, sono in grado di aumentare in modo significativo la fiducia del personale sanitario nel saper affrontare situazioni e comportamenti aggressivi nei vari setting operativi, permettendo una riduzione di fenomeni come il burnout, il turnover o l’abbandono dalla professione. Data l’entità del fenomeno della violenza e aggressività in ambito sanitario in crescente aumento, spesso poco o per nulla segnalate dagli operatori coinvolti, alcune evidenze suggeriscono di estendere i corsi di formazione sull’utilizzo delle tecniche di de-escalation non solo ai professionisti sanitari ma anche agli studenti afferenti ai vari Corsi di Studio, come quello in infermieristica.
