Introduction: The Family and Community Nurse (FCN) plays a core role in community-based healthcare, particularly in addressing loneliness among the elderly population. In the context of progressive demographic aging, the recognition and proper management of loneliness have become a priority in promoting active and healthy aging. This paper aims to analyze the main competencies of the FCN, focusing on soft skills, in order to understand their impact in building an effective relationship-centered care with the elderly patient experiencing loneliness. Objective: To understand the role of soft skills in the relationship-centered care between the nurse and the elderly patient, and to further explore the relational competencies of the Family and Community Nurse (FCN) in managing loneliness. Materials and Methods: A literature review has been conducted using PubMed search engine, employing three query strings composed of combined keywords. Articles published in the last 10 years, in Italian or English, and related to patients aged 65 and over, were selected. Results: After excluding non-eligible articles, 18 articles were selected, analyzed and summarized in a table. The evidence shows that loneliness represents a real issue with serious health consequences. The role of the FCN is further defined by considering its competencies, suggesting that soft skills act as an important element in nursing education and practice. Conclusions: Empathy, active listening, leadership and effective communication emerged as the key soft skills of the FCN for the relationship-centered care. It is highlighted that loneliness is constantly increasing due to changes in the social and family context, making enhanced community-based assistance crucial and positioning the FCN as a key figure. Critical concerns persist, regarding the elevated patient to nurse ratio and the need for more comprehensive research on paper related topics.
