Post-stroke aphasia may persist for more than six months after the acute event, with significant repercussions on social participation and quality of life, not only for the person directly affected but also for the caregiver who supports their communication daily basis. Approaches such as Communication Partner Training (CPT), an environmental rehabilitation intervention, have proven effective in enhancing the communication partner's skills and creating a more accessible context. This cross-sectional observational study, conducted at the outpatient speech and language therapy service of the Physical Medicine and Rehabilitation Unit at Jesi Murri Hospital (AN), involved four dyads consisting of individuals with chronic aphasia and their respective communication partner. The aim was to evaluate the effectiveness of CPT in improving the caregiver's communication competence and supporting the communicative dyad. The project took place from April to October 2025 and included a baseline assessment (T0), seven CPT conversational practice sessions with video recording and feedback, followed by a final reassessment (T1). The assessment included a set of measures administered both to the person with aphasia and to the caregiver, in order to investigate neuropsychological functions, functional communication, the dyad’s conversational ability, and the quality of life of both the person with aphasia and the caregiver. The results show an increase in caregivers’ communicative competences, with greater use of verbal, non-verbal, and paraverbal facilitative strategies, and a heterogeneous improvement in conversational participation and in perceived communication effectiveness for people with aphasia. Overall, the findings suggest that CPT may help to modify the everyday communication environment, promoting the use of the person with aphasia’s residual resources and their wellbeing and social participation in the chronic phase.
L’afasia post-ictus può persistere oltre sei mesi dall’evento acuto, determinando importanti ripercussioni sulla partecipazione sociale e sulla qualità di vita non solo della persona direttamente coinvolta, ma anche del caregiver che ogni giorno ne sostiene la comunicazione. Approcci come il Communication Partner Training (CPT), un intervento riabilitativo di tipo ambientale, si sono dimostrati efficaci nel potenziare le competenze del partner comunicativo e nel creare un contesto più accessibile. Il presente studio osservazionale trasversale (cross-sectional), condotto presso il servizio ambulatoriale di logopedia dell’Unità Operativa di Medicina Fisica e Riabilitazione dell’Ospedale Murri di Jesi (AN), ha coinvolto quattro diadi composte da persona con afasia cronica e rispettivo partner comunicativo, con l’obiettivo di valutare l’efficacia del CPT nel migliorare la competenza comunicativa del caregiver e nel supportare la diade comunicativa. Il progetto si è svolto da aprile ad ottobre 2025 e ha previsto una valutazione iniziale (T0), sette sedute di pratica conversazionale CPT con videoregistrazione e feedback, seguite da una rivalutazione finale (T1). La valutazione comprendeva un insieme di strumenti rivolti sia alla persona con afasia sia al caregiver, per indagare le funzioni neuropsicologiche, la comunicazione funzionale, l’abilità conversazionale della diade e la qualità di vita sia della persona con afasia che del caregiver. I risultati mostrano un incremento delle competenze comunicative dei caregiver, con un maggior uso di strategie facilitanti verbali, non verbali e paraverbali e un miglioramento eterogeneo della partecipazione alla conversazione e della percezione di efficacia comunicativa nelle persone con afasia. Nel complesso, i dati suggeriscono che l’intervento di CPT possa contribuire a modificare l’ambiente comunicativo quotidiano, favorendo l’uso delle risorse residue della persona con afasia e promuovendo il suo benessere e la sua partecipazione sociale nella fase cronica.
