Background: In the field of musculoskeletal conditions, a paradigm shift towards patient-centered practice that embraces the biopsychosocial model is observed in physiotherapy practice. Cognitive Functional Therapy (CFT) is proposed as a multidimensional treatment approach based on this model, the efficacy of which is being studied in a growing number of publications. Objective: The aim of the scoping review was to investigate the efficacy of CFT in the management of musculoskeletal conditions and to explore the evidence in order to highlight limitations and implications for clinical practice and future research. Materials and methods: A literature search was conducted from November 1 to November 30, 2025, in three biomedical databases (MEDLINE via PubMed, EPIESTEMONIKOS, and the Cochrane Library). A search string was generated for each database, and study selection was guided by predetermined inclusion/exclusion criteria. Results: The literature search yielded seven manuscripts (five systematic reviews and two RCTs). The most studied condition is low back pain, followed by neck pain. Several studies support its efficacy in the treatment of low back pain, particularly with regard to disability outcomes. If demonstrated, efficacy will be maintained at a three-year follow-up. Regarding neck pain, the evidence currently available is limited, although it reports positive results. Conclusions: The studies analyzed demonstrate the efficacy of CFT in the treatment of low back pain, particularly with regard to disability. As demonstrated by two RCTs, the effects appear to be maintained even at a three-year follow-up. The evidence supporting CFT in the treatment of neck pain is promising, but given the small number of published studies (two RCTs and one case report, with fewer than one hundred subjects studied), further studies with larger sample sizes are needed.
Background: Nell’ambito delle condizioni muscoloscheletriche si osserva nella pratica fisioterapica un cambio di paradigma verso una pratica centrata sul paziente che abbraccia il modello biopsicosociale. La Cognitive Functional Therapy (CFT) si propone come approccio di cura multidimensionale fondato su tale modello, la cui efficacia è oggetto di studio in un numero crescente di pubblicazioni; Obiettivo: L’obiettivo della scoping review era investigare l’efficacia della CFT nella gestione delle condizioni muscoloscheletriche ed esplorare le prove con il fine di mettere alla luce limitazioni ed implicazioni per la pratica clinica e la ricerca futura; Materiali e metodi: Una ricerca della letteratura è stata condotta dal 1 Novembre al 30 Novembre 2025 in 3 databases biomedici (MEDLINE tramite PubMed, EPIESTEMONIKOS e Cochrane Library). Per ogni singolo database è stata generata una stringa di ricerca e la selezione degli studi è stata guidata da criteri di inclusione/esclusione predeterminati; Risultati: La ricerca bibliografica ha portato il reperimento di 7 manoscritti (relativi a 5 revisioni sistematiche e 2 RCT). La condizione maggiormente studiata è il Low Back Pain, a seguire il Neck Pain. Nell’ambito del Low Back Pain sono presenti diversi studi che ne sostengono l’efficacia, in particolare in riferimento all’outcome disabilità. L’efficacia si è dimostrata mantenersi a 3 anni di follow-up. Per quanto riguarda il Neck Pain le prove ad oggi disponibili sono scarse seppur riportano risultati positivi; Conclusioni: Gli studi analizzati dimostrano l’efficacia della CFT in ambito di Low Back Pain, in particolare, nei confronti della disabilità. Come dimostrato da 2 RCT gli effetti sembrano mantenersi anche al follow-up di 3 anni. Le prove a sostegno della CFT nel trattamento del Neck Pain sono promettenti, ma dato l’esiguo numero di studi pubblicati (2 RCT e un case report, con meno di cento soggetti studiati) sono necessari ulteriori studi su campioni più ampi di soggetti.
