Background Optimizing Quality of Life (QoL) is currently considered one of the treatment endpoints for inflammatory bowel disease (IBD), alongside the normalization of clinical-laboratory and endoscopic parameters. Various questionnaires exist to measure this multidimensional parameter; however, most are rather long and complex (e.g., IBDQ-32) and therefore difficult to apply in daily clinical practice. The validation of the IBD-Control-8 (IBD-C-8) aims to assess, in a brief manner using only 8 items, the impact of IBD on the patient's QoL, considering not only intestinal symptoms but also the emotional sphere and social functioning. A total score ≥ 13 indicates optimal IBD control and a good QoL. Objectives To evaluate, using the IBD-C-8, the prevalence of poor QoL (cut-off < 13) in a cohort of patients with ulcerative colitis (UC) or Crohn's disease (CD) attending the IBD Unit of the Azienda Ospedaliera Universitaria delle Marche in Ancona. Secondary objectives include assessing discrepancies in QoL based on the type of disease (UC vs. CD) or current therapy (standard vs. advanced); the correlation between the score and the patient's clinical or biohumoral profile; the concordance between the therapeutic changes proposed during the visit and the patient's perception of their ongoing therapy; and the prevalence of fatigue. Study A cross-sectional observational study was conducted on 65 patients with IBD (56.9% CD, 43.1% UC) visited at the IBD Unit between February and May 2026. During the visits, the IBD-C-8 was administered, and parameters regarding disease phenotypes and biomarkers most closely correlated with disease activity (C-reactive protein, hemoglobin, and fecal calprotectin) were recorded. The non-Gaussian scores were expressed as Median and IQR and analyzed using the Mann-Whitney, Chi-square, or Fisher's Exact tests. Results The median IBD-C-8 score was 14.0 (IQR: 10.0-16.0): 63.1% of patients showed good QoL (≥ 13) and 36.9% showed poor QoL. No significant differences in QoL emerged between UC and CD (Median 14.0 vs 13.5; p > 0.05). No difference in QoL was observed between standard and advanced therapy (Median 14.0 for both; p > 0.05), the latter being administered in a higher percentage of patients with CD (57.1% vs. 27.0%; p = 0.0106). The group with poor QoL (< 13) showed double the rates of steroid use in the previous 6 months (25.0% vs. 12.2%), a higher association with complex extraintestinal manifestations (EIMs), and perianal surgical phenotypes. In 29.3% of patients with optimal QoL (≥ 13), Fecal Calprotectin was > 200μg/g of stool, indicating silent subclinical inflammation despite the good perceived QoL. Chronic fatigue (62.5% in scores < 13) was confirmed as the main negative driver of the score. Conclusions The IBD-C-8 proves to be an agile and effective tool for QoL screening in the outpatient setting. The data highlight that QoL is minimally influenced by the type of disease or therapy received, suggesting that advanced therapies provide a benefit even in IBD with a more aggressive course. Certain parameters, such as fatigue and anxiety, have an impact independent of the phenotype and objective or perceived disease activity, indicating that the integration of multiple parameters—not only biohumoral ones—is essential to achieve a holistic and comprehensive remission.

Background: L'ottimizzazione della Qualità della Vita (QoL) è oggi considerata uno degli endpoint del trattamento delle malattie infiammatorie croniche intestinali (MICI) al fianco della normalizzazione di parametri clinico-laboratoristici ed endoscopici. Per misurare questo parametro multidimensionale esistono diversi questionari, gran parte dei quali piuttosto lunghi e complessi (es. IBDQ-32) e dunque di difficile applicabilità nella pratica clinica quotidiana. La validazione dell’IBD-Control-8 (IBD-C-8) cerca di valutare in maniera breve, con soli 8 item, l’impatto che la MICI ha sulla QoL del paziente considerando non solo i sintomi intestinali, ma anche la sfera emozionale e il funzionamento sociale. Uno score totale ≥ 13 indica un controllo ottimale della MICI e una buona QoL. Obiettivi: Valutare tramite l'IBD-C-8 la prevalenza di una scarsa QoL (cut-off < 13) in una coorte di pazienti affetti da rettocolite ulcerosa (RCU) o malattia di Crohn (MC) afferenti all’IBD Unit dell’Azienda Ospedaliero Universitaria delle Marche di Ancona. Valutare secondariamente la presenza di discrepanze della QoL in base al tipo di patologia (RCU o MC) o al tipo di terapia in atto (standard vs avanzata), la correlazione tra punteggio dello score e il profilo clinico o bioumorale del paziente, la concordanza tra le modifiche terapeutiche proposte alla visita e la percezione della terapia in atto da parte del paziente, la prevalenza dell’affaticamento. Studio: Studio osservazionale trasversale condotto su 65 pazienti con MICI (56,9% MC, 43,1% RCU) visitati presso l'IBD Unit tra febbraio e maggio 2026. Durante le visite è stato somministrato l'IBD-C-8 e sono stati registrati parametri riguardanti i fenotipi di malattia e i biomarcatori maggiormente correlati all’attività di malattia (proteina C reattiva, l’emoglobina e la Calprotectina fecale). I punteggi, non gaussiani, sono stati espressi come Mediana e IQR e analizzati con i test di Mann-Whitney, Chi-quadrato o Esatto di Fisher. Risultati: La mediana dell'IBD-C-8 è risultata di 14.0 (IQR: 10.0-16.0): il 63,1% dei pazienti ha mostrato una buona QoL (≥13) e il 36,9% una scarsa QoL. Non sono emerse differenze significative nella QoL tra RCU e MC (Mediana 14.0 vs 13.5; p >0.05). Nessuna differenza della QoL è emersa tra terapia standard e avanzata (Mediana 14.0 per entrambe; p>0.05), effettuata in percentuale maggiore nei pazienti con MC (57,1% vs 27,0%; p=0.0106). Il gruppo con scarsa QoL (<13) ha mostrato tassi doppi di uso di steroidi nei 6 mesi precedenti (25,0% vs 12,2%), maggiore associazione a manifestazioni extraintestinali (MEI) complesse e fenotipi chirurgici perianali. Nel 29,3% dei pazienti con ottima QoL (≥13) la Calprotectina Fecale risultava > 200 µg/g di feci, spia di flogosi subclinica silente nonostante la buona QoL percepita. Fatigue cronica (62,5% negli score <13) si conferma il principale driver negativo dello score. L'IBD-C-8 risulta uno strumento agile ed efficace per lo screening della QoL in ambulatorio. I dati evidenziano come la QoL sia poco influenzata dal tipo di malattia o di terapia effettuata, suggerendo come le terapie avanzate vadano ad apportare un beneficio anche nelle MICI con decorso più aggressivo. Alcuni parametri, come la fatigue e l'ansia, hanno un impatto indipendente dal fenotipo e dall’attività oggettiva o percepita di malattia, indicando come l’integrazione di più parametri, non solo bioumorali, siano fondamentali per raggiungere una remissione olistica e complessiva.
