Scalp reconstruction represents a complex surgical challenge due to the limited elasticity of the tissues, the presence of the galea aponeurotica, and the need to achieve adequate aesthetic and functional outcomes. This study evaluates the clinical applicability, safety, and effectiveness of the O-S flap, a novel surgical technique proposed for the reconstruction of small- to medium-sized scalp defects. A single-center retrospective study with an experimental purpose was conducted on 8 patients who underwent scalp reconstruction using this technique. The study population included 7 males and 1 female, with a mean age of 71.9 years. Defect sizes ranged from 7 to 78.5 cm². Complete wound healing was achieved in 100% of patients. No cases of necrosis or infection were observed. One minor wound dehiscence occurred and was successfully treated with secondary outpatient resuturing. The mean operative time was approximately 60 minutes. These preliminary results suggest that the O-S flap may represent a safe, effective, and reproducible technique for the reconstruction of small- to medium-sized scalp defects, although further studies on larger patient cohorts are required.
La ricostruzione dei difetti del cuoio capelluto rappresenta una sfida chirurgica complessa a causa delle caratteristiche anatomiche e biomeccaniche di questa regione, tra cui la scarsa elasticità dei tessuti, la presenza della galea aponeurotica e la necessità di ottenere un adeguato risultato estetico e funzionale. Il presente lavoro valuta l’applicabilità clinica, la sicurezza e l’efficacia del lembo O-S, una nuova tecnica chirurgica proposta per la ricostruzione dei difetti medio-piccoli dello scalpo. È stato condotto uno studio retrospettivo monocentrico con finalità sperimentale su 8 pazienti sottoposti a ricostruzione del cuoio capelluto mediante tale tecnica. La casistica comprendeva 7 uomini e 1 donna, con età media di 71,9 anni. Le dimensioni dei difetti trattati erano comprese tra 7 e 78,5 cm². La guarigione completa è stata ottenuta nel 100% dei pazienti. Non sono stati osservati casi di necrosi né di infezione. È stata registrata una sola deiscenza minore, trattata con risutura secondaria ambulatoriale e risolta senza ulteriori complicanze. Il tempo operatorio medio è stato di circa 60 minuti. I risultati ottenuti suggeriscono che il lembo O-S possa rappresentare una tecnica sicura, efficace e riproducibile per la ricostruzione dei difetti medio-piccoli del cuoio capelluto, pur richiedendo ulteriori studi su casistiche più ampie.
