Background. The ageing of the Italian population, one of the longest-living worldwide, is associated with an increase in frailty, a geriatric syndrome characterized by reduced physiological reserve and greater vulnerability to adverse events, leading to a higher risk of disability, hospitalization, and mortality. Since frailty is potentially reversible, early identification is essential. However, validated tools such as the Fried Frailty Phenotype and the Edmonton Frail Scale have limitations related to subjectivity and low sensitivity in detecting subclinical neuromuscular alterations. Wearable technologies integrating surface electromyography and accelerometry may provide objective biomarkers of motor function. Within this framework, the O FRAIL pilot study was designed to evaluate the use of the REMO device, which combines sEMG and an accelerometer, as an instrumental support to the clinical assessment of frailty. Objectives. The primary objective was to assess the clinical and operational feasibility of using REMO during the Handgrip Test, the Ten Meter Walk Test, and the Five Chair Stand Test, integrated with frailty assessment through the Fried Phenotype and the Edmonton Frail Scale. The secondary objective was to explore associations between neuromuscular and kinematic parameters acquired by the device and the outcomes of standardized clinical scales. Methods. This pilot observational study, cross sectional and single centre, was conducted on ten community dwelling older adults. During the three functional tests, sEMG signals and inertial data were collected simultaneously using REMO, positioned on the dominant upper and lower limbs. Feasibility was evaluated in terms of technical validity of the acquisitions, adherence to the protocol, operational timing, tolerability, usability, and acceptability. Results. According to the Edmonton Frail Scale, all participants were non frail, whereas the Fried Phenotype classified four as robust, five as pre frail, and one as frail. All acquisitions were valid on the first attempt, with no adverse events. Mean sensor placement time was 3.65 minutes, and instrumental assessment time was 19.89 minutes. Usability and acceptability were satisfactory. Exploratory analyses showed associations consistent with clinical constructs: greater grip strength was associated with higher sEMG amplitude, the device estimated walking time showed moderate correlation with clinical measurement, and in the Five Chair Stand Test longer sit to stand times and reduced muscle activation were associated with higher Fried scores. Conclusions. The use of REMO in frailty assessment proved feasible, well tolerated, and compatible with clinical practice. Despite the small sample size and the exploratory nature of the study, sEMG and inertial parameters emerge as potential digital biomarkers for a more objective evaluation of physical vulnerability. Larger and longitudinal studies are needed to confirm their reliability and clinical applicability.

L’invecchiamento della popolazione comporta un aumento della fragilità, sindrome geriatrica caratterizzata da ridotta riserva fisiologica e maggiore vulnerabilità agli eventi avversi, con conseguente incremento del rischio di disabilità, ospedalizzazione e mortalità. Poiché la fragilità è potenzialmente reversibile, una identificazione precoce è essenziale. Strumenti validati come il Fenotipo di Fried e la Edmonton Frail Scale presentano limiti legati alla soggettività e alla scarsa sensibilità verso alterazioni neuromuscolari subcliniche. Tecnologie indossabili che integrano elettromiografia di superficie e accelerometria possono offrire biomarcatori oggettivi della funzione motoria. In questo contesto si colloca lo studio pilota O FRAIL, volto a valutare l’impiego del dispositivo REMO, che combina sEMG e accelerometro, come supporto alla valutazione clinica della fragilità. Obiettivi. Valutare la fattibilità clinico operativa dell’uso di REMO durante Handgrip Test, Ten Meter Walk Test e Five Chair Stand Test, e esplorare le associazioni tra parametri neuromuscolari e cinematici e gli esiti delle scale cliniche. Metodi. Studio osservazionale pilota, trasversale, monocentrico, condotto su dieci anziani di comunità. Durante i tre test funzionali sono stati acquisiti simultaneamente segnali sEMG e dati inerziali tramite REMO, posizionato su arto superiore e inferiore dominanti. La fattibilità è stata valutata considerando validità tecnica, aderenza al protocollo, tempi operativi, tollerabilità, usabilità e accettabilità. Risultati. Secondo Edmonton tutti i partecipanti erano non fragili, mentre Fried ha classificato quattro robusti, cinque pre fragili e uno fragile. Tutte le acquisizioni sono risultate valide al primo tentativo, senza eventi avversi. Il posizionamento dei sensori ha richiesto in media 3.65 minuti e la valutazione strumentale 19.89 minuti. Usabilità e accettabilità sono risultate soddisfacenti. Le analisi esplorative hanno mostrato associazioni coerenti con i costrutti clinici: maggiore forza di presa associata a maggiore ampiezza sEMG, correlazione moderata tra tempo di cammino stimato e misurazione clinica, e nel Five Chair Stand Test tempi più lunghi e riduzione dell’attivazione muscolare associati a punteggi più elevati del Fenotipo di Fried. Conclusioni. L’uso di REMO nella valutazione della fragilità è risultato fattibile, tollerato e compatibile con la pratica clinica. Nonostante la numerosità limitata e la natura esplorativa, i parametri sEMG e inerziali emergono come potenziali biomarcatori digitali per una valutazione più oggettiva della vulnerabilità fisica. Studi più ampi e longitudinali sono necessari per confermarne affidabilità e applicabilità clinica.
