Chronic kidney disease (CKD) is associated with a high incidence of dysgeusia, which may contribute to reduced dietary adherence and malnutrition. The aim of this thesis was to evaluate and compare taste perception in three cohorts of CKD patients — hemodialysis (HD), kidney transplant recipients (TX), and advanced CKD patients (MRA) — using the modified filter paper strip method; moreover, the relationship between taste sensitivity and selected serum parameters (potassium, phosphorus, calcium, urea) and salivary parameters (uric acid, ORAC, ROS, SOD) was investigated. Among the 51 enrolled patients (HD n = 22, TX n = 20, MRA n = 9), the HD group showed the lowest percentage of correct responses (38%; umami and water were the most impaired tastes), followed by the MRA group (56%) and the TX group (62.7%). Taste perception was negatively correlated with serum potassium, phosphorus, and urea, and positively correlated with serum calcium and salivary uric acid, the latter likely related to its antioxidant function in the oral cavity. Salivary uric acid was also significantly higher in the MRA group than in TX and HD. The results confirm that kidney transplantation is associated with a significant recovery. Dysgeusia in CKD is confirmed as a multifactorial phenomenon, and its assessment may help identify patients at early nutritional risk.

La malattia renale cronica (CKD) è associata a un'elevata incidenza di disgeusia, che può contribuire a ridotta aderenza dietetica e malnutrizione. Lo scopo della tesi è stato valutare e confrontare la percezione gustativa in tre coorti di pazienti con CKD - in emodialisi (HD), con trapianto di rene (TX), e CKD allo stadio avanzato (MRA) - mediante il metodo delle filter paper strip modificato; inoltre, è stata indagata la relazione della sensibilità gustativa con alcuni parametri sierici (potassio, fosforo, calcio, urea) e salivari (acido urico, ORAC, ROS, SOD). I risultati hanno mostrato che, dei 51 pazienti arruolati (HD n=22, TX n=20, MRA n=9), il gruppo HD ha mostrato la percentuale di risposte corrette più bassa (38%; umami e acqua risultavano i gusti più compromessi), seguito dal gruppo MRA (56%) e TX (62,7%). La percezione gustativa è correlata negativamente con potassio, fosforo e urea sierici, e positivamente con calcio sierico e acido urico salivare, quest'ultimo probabilmente legato alla sua funzione antiossidante nel cavo orale. L'acido urico salivare è risultato inoltre significativamente più elevato nel gruppo MRA rispetto a TX e HD. I risultati confermano che il trapianto renale si associa a un recupero significativo. La disgeusia in CKD si conferma un fenomeno multifattoriale, la cui valutazione potrebbe favorire l'identificazione precoce dei pazienti a rischio nutrizionale.
