BACKGROUND: Italy represents one of the top European markets in the field of central venous access with peripheral insertion. In fact, as reported by iData research (2016), the PICCs used annually are 82,371. The number of PICCs produced corresponds to the number of PICCs actually purchased by hospitals and implanted. All this demonstrates the fact that the PICC is establishing itself in the Italian and non-Italian healthcare scenario, as a safe and efficient vascular access. INTRODUCTION: the study was born from the internship experience carried out at the PICC UNIT and from the statistical data that nowadays demonstrate the centrality of the use of the PICC. The emergency due to the Covid-19 pandemic has also strengthened the idea of developing the study. OBJECTIVE OF THE THESIS: to observe and evaluate whether it is useful for patients selected for the study to carry out self-medication of the unit carried out by a caregiver at home with the necessary information found in the brochure. The information brochure for the patient and caregiver was created on the basis of the most recent international recommendations and guidelines; it is to be used as an information support for self-medication, but after an adequate training course for the caregiver carried out by experienced nursing staff. Also evaluate the comprehension, completeness and any advice for the purpose of improving the brochure. MATERIALS AND METHODS: 50 patients with PICC were consecutively selected between August and September 2020 at: PICC UNIT and S.O.D. of Hematology and Oncology of the University Hospital of Ospedali Riuniti of Ancona and each patient was provided with an explanatory leaflet relating to the various management procedures of the hospital and a questionnaire with related questions. On the basis of the answers obtained from the anonymous questionnaires, the various objectives of the thesis were evaluated. RESULTS: with a response rate of 90% it emerged that medicating the PICC at home following the Covid-19 pandemic is useful for 80% of patients; however, in general 56% of them are in favor of home medication. The brochure is clear and comprehensive to 91% of the study participants. Only 29% of them consider it appropriate to add explanatory information to the brochure and 31% explicitly propose advice for its improvement. CONCLUSIONS: despite the clarity of the brochure, a small percentage of patients are not yet willing to carry out the PICC home medication. This underlines the importance of providing adequate training for the caregiver as a support to the information in the brochure and of guaranteeing the figure of the caregiver to patients who do not have one.

Il numero di PICC prodotti corrisponde al numero di PICC effettivamente comprati dalle strutture ospedaliere ed impiantati. Tutto ciò a dimostrazione del fatto che, il PICC si sta affermando nello scenario sanitario italiano e non, in quanto accesso vascolare sicuro ed efficiente. INTRODUZIONE: lo studio nasce dall’esperienza di tirocinio effettuata presso la PICC UNIT e dai dati statistici che dimostrano al giorno d’oggi la centralità dell’utilizzo del PICC. L’emergenza dovuta alla pandemia di Covid-19 ha inoltre rafforzato l’idea di sviluppare lo studio. OBIETTIVO DELLA TESI: osservare e valutare se ai pazienti selezionati per lo studio risulti utile effettuare l’automedicazione del presidio effettuata da un caregiver a casa con le informazioni necessarie reperite nell’opuscolo. L’opuscolo informativo per il paziente ed il caregiver è stato realizzato sulla base delle più recenti raccomandazioni e linee guida internazionali; è da usare come supporto informativo per l’automedicazione, ma dopo un adeguato percorso formativo per il caregiver effettuato da personale infermieristico esperto. Inoltre valutare la comprensione, l’esaustività ed eventuali consigli ai fini del miglioramento dell’opuscolo. MATERIALI E METODI: sono stati selezionati in maniera consecutiva 50 pazienti portatori di PICC tra Agosto e Settembre 2020 presso: PICC UNIT e S.O.D. di Ematologia ed Oncologia dell’Azienda Ospedaliero Universitaria Ospedali Riuniti di Ancona e ad ogni paziente è stato fornito un opuscolo esplicativo relativo alle varie procedure di gestione del presidio ed un questionario con relative domande. Sulla base delle risposte ottenute dai questionari in forma anonima si sono andati a valutare i vari obiettivi della tesi. RISULTATI: con un response rate del 90% è emerso che medicare a casa il PICC in seguito alla pandemia di Covid-19 è utile all’80% dei pazienti; comunque in generale il 56% di loro è favorevole alla medicazione domiciliare. L’opuscolo risulta chiaro ed esaustivo al 91% dei partecipanti allo studio. Solo il 29% di loro ritiene opportuno aggiungere informazioni esplicative all’opuscolo ed il 31% propone esplicitamente consigli per il suo miglioramento. CONCLUSIONI: nonostante la chiarezza dell’opuscolo, una piccola percentuale di pazienti non risulta ancora disposta ad effettuare la medicazione domiciliare del PICC. Questo sottolinea l’importanza di effettuare un’adeguata formazione al caregiver come supporto alle informazioni presenti nell’opuscolo e di garantire la figura del caregiver ai pazienti che non ne dispongono.

The management of Peripherally Inserted Central Catheter (PICC), the patient as key player. Observational study.
