ABSTRACT Background With the increase in the average life expectancy of the population, chronic degenerative diseases as well as oncological diseases are constantly increasing, and this leads health professionals, particularly nurses, to increasingly confront the end of life of patients. . Precisely for this reason, it is very important to deal with and analyze nursing care in a dying patient in a precise and timely manner. The approach to the person is certainly multidisciplinary, in this paper we focus on nursing care, on what palliative care is today and the rules that regulate it, as well as the psychological aspect that is experienced both from a professional and personal point of view. addressing these end-of-life paths. Aims The aim of the thesis work is to analyze the role of the nurse in assisting the patient in the terminal stages of life, and the best solutions to make this path as dignified as possible. Methods The bibliographic search was carried out by consulting databases such as PubMed, using a search string built with the PICOM method. The selection of the articles was made using the following inclusion criteria: • Population: Patients in the terminal stages of life • Intervention: Nursing Assistance • Outcome measures: Best possible nursing care, better patient quality of life, better interventions • Characteristics of the studies: English or Italian, literature published in the last 5 years Results By excluding duplicates, irrelevant studies, referencing the literature from the last five years, and increasing the number of relevant keywords, 55 articles were found that met the inclusion criteria, of which 15 were selected. Conclusions The studies and articles analyzed in this review underline and support the importance of nursing in a holistic view. In particular, it is not only the control of the patient's physical symptoms, in particular pain, to accompany the patient in the last stages of life, but also the psychological support of the person, and of the family itself in a holistic vision, of a path designed as a tailored suit on the life and needs of each person who faces the path of the end of life. One aspect that emerged is that which places Italy as a country only at the beginning of the culture of the end of life, of which unfortunately very little is still spoken about, although progress has been made, the growing number of chronically ill and terminals makes it urgent to establish a priority in talking about this issue and to implement a care culture of palliation.
ABSTRACT Background Con l’aumentare dell’aspettativa di vita media della popolazione, le malattie cronico degenerative nonché le patologie oncologiche sono in costante aumento, e questo porta i professionisti sanitari, in modo particolare gli infermieri, a confrontarsi sempre più spesso con il fine vita dei pazienti. Proprio per questo l’affrontare e analizzare in maniera precisa e puntuale l’assistenza infermieristica nel paziente terminale è importantissimo. L’approccio alla persona è sicuramente multidisciplinare, in questo elaborato ci si concentra sull’assistenza infermieristica, su cosa sono le cure palliative oggi e le norme che le regolano, nonché l’aspetto psicologico che si vive sia dal punto di vista professionale che personale affrontando questi percorsi di fine vita. Obiettivi Obiettivo del lavoro di tesi è analizzare il ruolo dell’infermiere nell’assistenza al paziente nelle fasi terminali della vita, e le soluzioni migliori per rendere questo percorso il più possibile dignitoso. Metodi La ricerca bibliografica è stata effettuata consultando banche dati quali PubMed, usando una stringa di ricerca costruita con il metodo PICOM. La selezione degli articoli è avvenuta usando i seguenti criteri di inclusione: • Popolazione: Pazienti nelle fasi terminali della vita • Intervento: Assistenza Infermieristica • Misure di esito: Migliore assistenza infermieristica possibile, migliore qualità di vita del paziente, migliori interventi • Caratteristiche degli studi: lingua inglese o italiana, letteratura pubblicata negli ultimi 5 anni Risultati Escludendo i duplicati, gli studi non pertinenti, prendendo in riferimento la letteratura degli ultimi cinque anni, e incrementando il numero di parole chiave pertinenti, sono stati reperiti 55 articoli che soddisfacevano i criteri di inclusione, tra i quali ne sono stati selezionati 15. Conclusioni Gli studi e gli articoli analizzati in questa revisione sottolineano e supportano l’importanza dell’assistenza infermieristica in una visione olistica. In particolare non è fondamentale solamente il controllo dei sintomi fisici del paziente, in modo particolare il dolore, per accompagnare il paziente nelle ultime fasi della vita, ma anche il supporto psicologico alla persona, e della famiglia stessa in una visione olistica, di un percorso disegnato come un abito sartoriale sulla vita ed i bisogni di ogni persona che affronta il cammino del fine vita. Un aspetto che è emerso, è quello che colloca l’Italia come paese solo agli inizi della cultura del fine vita, della quale se ne parla purtroppo ancora molto poco, per quanto siano stati fatti dei passi avanti, il numero crescente di malati cronici e terminali rende urgente stabilire una priorità nel parlare di questa tematica ed attuare una cultura assistenziale della palliazione.
