Introduction: In recent years Sacral Neuromodulation (NMS) has proven to be an alternative that can be implemented in patients who do not respond to common therapeutic treatments, effective in the treatment of various dysfunctional diseases affecting the lower urinary tract and recently has also proved useful in the treatment of abnormalities involving the intestinal tract. The problems caused by these dysfunctions do not only affect the physical environment of those who suffer from them but also the psychological sphere, so much so as to affect the different daily activities that people face, so as to alter their quality of life. Objective: To detect through the administration of the SF-36 questionnaire if Sacral Neuromodulation allows to take advantage also in terms of quality of life. Materials and methods: The quality of life (QoL, Quality of Life) was evaluated, through the administration of the SF-36 questionnaire, of 10 individuals (9 females and 1 male) aged between 32 and 72 years who were suffering from urinary and defecatory diseases (urinary incontinence, urinary retention, urgency/frequency syndrome, fecal incontinence, constipation and chronic pelvic pain) and who underwent sacral neuromodulation. To determine whether the therapeutic treatment contributed to improving the quality of life of the subjects, a comparison was made before and after surgery. The questionnaire was completed, through self-assessment and by telephone, from February 2020 until September of the same year, with total respect for privacy and subject to patient consent. The SF-36 consists of 36 questions related to 8 dimensions of health: Physical Activity (AF), Role Limitations and Physical Health (RF), Physical Pain (DF), General Health (SG), Vitality (VT), Social Activity (AS), Role Limitations and Emotional State (RE), Mental Health (MS). The scores are standardized in numerical scales from a minimum of 0, which indicates poor health, to a maximum of 100, which represents optimal health. Results: The scores attributable to the domains of the pre-intervention questionnaire are almost all below the average value (50), the only size that is slightly above this value is the variable MS, which determines that the health of the subjects was directed towards a very poor health, while, the scores attributable to the size of the post-intervention questionnaire are mostly above the average value (50), only the SG domain remains slightly below this value. Specifically, the SM, RE, AS, VT, RF scales showed an important improvement (p < 0.050). Instead the AF, DF and SG domains are on average equal before and after surgical treatment (p > 0.050). Conclusions: The average QoL scores of the 8 dimensions were exhaustive, this indicates that patients find slight difficulties in work or other daily activities, have a fair physical health even if a portion of the sample believes that it is not like the others. Patients perform all types of activities, including the most demanding ones even if partially limited, without having particular physical or emotional difficulties, since the perceived pain is not so intense as to limit their performance; finally, their mood is largely positive. Therefore, the sacral neuromodulation can represent a valid choice for patients who are candidates.

Introduzione: Negli ultimi anni la Neuromodulazione Sacrale (NMS) si è rivelata un’alternativa che può essere attuata in pazienti che non rispondono ai comuni trattamenti terapeutici, efficace nel trattamento di diverse patologie disfunzionali che interessano il basso tratto urinario e di recente si è dimostrata utile anche nei confronti delle anomalie che coinvolgono il tratto intestinale. Le problematiche causate da queste disfunzioni non interessano unicamente l’ambito fisico dei soggetti che ne soffrono ma anche la sfera psicologica, tanto da influire sulle diverse attività quotidiane che le persone si trovano ad affrontare, così da alterare la loro qualità di vita. Obbiettivo: Rilevare attraverso la somministrazione del questionario SF-36 se la Neuromodulazione Sacrale consente di trarre vantaggio anche in termini di qualità di vita. Materiali e metodi: È stata valutata la qualità di vita (QoL, Quality of Life), attraverso la somministrazione del questionario SF-36, di 10 individui (9 femmine e 1 maschio) con età compresa tra i 32 e 72 anni che erano affetti da patologie urinarie e defecatorie (incontinenza urinaria, ritenzione urinaria, sindrome urgenza/frequenza, incontinenza fecale, stipsi e dolore pelvico cronico) e che sono stati sottoposti a neuromodulazione sacrale. Per determinare se il trattamento terapeutico ha contribuito a migliorare la qualità di vita dei soggetti è stato eseguito un confronto prima e dopo l’intervento chirurgico. Il questionario è stato compilato, attraverso l’auto-valutazione e per via telefonica, a partire da febbraio 2020 fino a settembre dello stesso anno, nel rispetto totale della privacy e previo consenso da parte del paziente. L’SF-36 è costituito da 36 quesiti riconducibili ad 8 dimensioni della salute: Attività fisica (AF), limitazioni del Ruolo e salute fisica (RF), Dolore fisico (DF), Salute in Generale (SG), Vitalità (VT), Attività sociale (AS), limitazioni del Ruolo e stato emotivo (RE), Salute Mentale (SM). I punteggi vengono standardizzati in scale numeriche da un minimo di 0, che indica una salute pessima, ad un massimo di 100, che rappresenta una salute ottimale. Risultati: I punteggi attribuibili ai domini del questionario pre-intervento sono quasi tutte sotto il valore medio (50), l’unica dimensione che è leggermente sopra tale valore è la variabile SM, ciò determina che la salute dei soggetti era direzionata verso una salute pessima, mentre, i punteggi riconducibili alle dimensioni del questionario post-intervento sono prevalentemente sopra il valore medio (50), solamente il dominio SG rimane scarsamente al di sotto di tale valore. Nello specifico, le scale SM, RE, AS, VT, RF hanno mostrato un importante miglioramento (p < 0.050). Invece i domini AF, DF e SG sono mediamente uguali prima e dopo il trattamento chirurgico (p > 0.050). Conclusioni: I punteggi medi di QoL delle 8 dimensioni sono risultati esaurienti, ciò indica che i pazienti trovano lievi difficoltà nel lavoro o in altre attività quotidiane, hanno una discreta salute fisica anche se una porzione del campione ritiene che essa non sia come quella degli altri. I pazienti svolgono tutti i tipi di attività, comprese le più impegnative anche se limitate in modo parziale, senza avere particolari difficoltà fisiche o emotive, visto che il dolore percepito non è così intenso da limitarne l’esecuzione; infine, il loro stato d’animo è ampiamente positivo. Quindi la neuromodulazione sacrale può rappresentare una valida scelta per i pazienti che ne sono candidati.

Observational study on the quality of life of patients undergoing sacral neuromodulation.
