The over rule is a phenomenon that, although it is frequent and widespread in Emergency/Emergency Departments, has still been little analysed. At all times, there are triage nurses who find themselves having to work in an environment hostile to them, having to juggle prevarications of all kinds during the performance of their duties. Whether they are patients, doctors or colleagues, the triage nurse not only has the responsibility of assigning a code for priority access, but will also have to defend the reasons for his choice from those who do not consider him capable and trained enough to make such decisions. Objective of the study: to analyse the phenomenon of over rule, its causes and possible solutions to combat it. Materials and methods: data collection was carried out by filling in an anonymous questionnaire, which was submitted to the triagist nurses of all the ERs, through the Google Modules platform. Results: we received answers that show a very problematic picture, in which the continuous pressure exerted on the triage operators is causing a drop in motivation among the staff, a reason to decide to leave the triage activity, as well as a reason to change the codes previously assigned by the nurses. Conclusions: making triage nurses more aware of their role, of their mandate can contribute greatly to improving their ability to resolve these unpleasant situations, together with the establishment of protocols to reduce subjectivity, continuous training and team meetings to tackle all types of problems together.
L’over rule è un fenomeno che per quanto sia frequente e diffuso nei Dipartimenti di Urgenza-Emergenza, è stato ancora poco analizzato. In ogni momento ci sono infermieri di triage che si ritrovano a dover lavorare in un ambiente a loro ostile, dovendosi giostrare tra prevaricazioni di ogni genere durante il compimento del loro mandato. Che siano essi pazienti, medici o colleghi, l’infermiere triagista oltre ad avere la responsabilità di assegnare codice per la priorità di accesso, dovrà anche difendere le motivazioni della sua scelta da chi non lo ritiene capace e abbastanza formato per compiere decisioni del genere. Obbiettivo dello studio: analizzare il fenomeno dell’over rule, le cause e le possibili soluzioni per contrastarlo. Materiali e metodi: la raccolta dei dati è stata effettuata attraverso la compilazione di un questionario in forma anonima, sottoposto agli infermieri triagisti di tutti i Pronto Soccorso, attraverso la piattaforma Google Moduli. Risultati: abbiamo ricevuto risposte che mostrano un quadro molto problematico, in cui le continue pressioni esercitate sugli operatori di triage, sono causa di calo di motivazione tra il personale, motivo di decisione a lasciare l’attività di triage, nonché motivo per cambiare i codici precedentemente assegnati dagli infermieri. Conclusioni: rendere più consapevoli gli infermieri triagisti del loro ruolo, del loro mandato può contribuire enormemente a migliorare la capacità di risolvere queste spiacevoli situazioni, unitamente alla costituzione di protocolli che diminuiscano la soggettività, formazione continua e riunioni di equipe per affrontare insieme ogni tipologia di problema.
