RUOLO DELL'INFERMIERE NELLA TRASFUSIONE. COME EVITARE L'ERRORE E COSA COMPORTEREBBE ALL'ORGANISMO.

In this study I investigated the role of the nurse in transfusion practice, analyzing the errors that the nurse can make and how to prevent them. For this I conducted a literature review, using different search engines, and the terms (and relevant MESH terms) Patient, Blood Transfusion, Nursing Errors, Transfusion Reaction. The professional experience and expert opinion of the Transfusion Service of the Civil Hospital of Macerata were also used. Transfusion safety is an entire complex that includes all the activities of the transfusion chain ranging from the selection of the donor to the infusion of blood components in the recipient patient. The nurse plays an important role and is responsible for many steps during the transfusion including identifying the person prior to collection or administration. The errors it can run into are many,. I have analyzed from the literature the nursing errors that occur most frequently, trying to understand why and how it is possible to reduce them by introducing well-defined procedures. However, it should be borne in mind that it will never be possible to completely eliminate human error. Transfusion surveillance is the responsibility of the nurse, and knowledge of adverse reactions is lacking. Recognizing an adverse reaction and acting in time is important: just think of an acute haemolytic reaction due to ABO incompatibility for which a few drops are enough to put the patient's life at risk.

In questo studio ho approfondito il ruolo dell’infermiere nella pratica trasfusionale, analizzando gli errori in cui l’infermiere può incorrere e come poterli prevenire. Per questo ho condotto una revisione della letteratura, utilizzando diversi motori di ricerca, ed i termini (e termini MESH pertinenti) Patient, Blood transfusion, nursing errors, transfusion reaction. Sono state impiegate anche l’esperienza professionale e l’opinione di esperti del Servizio Trasfusionale dell’Ospedale Civile di Macerata. La sicurezza trasfusionale è un iter complesso che include tutte le attività della filiera trasfusionale che vanno dalla selezione del donatore all’infusione degli emocomponenti nel paziente ricevente. L’infermiere ha un ruolo importante ed è responsabile di molte fasi durante la trasfusione tra cui l’identificazione della persona prima del prelievo o prima dell’avvenuta somministrazione. Pertanto gli errori in cui può incorrere sono molteplici,. Ho analizzato dalla letteratura gli errori infermieristici che avvengono con più frequenza, cercando di capire il perché e come è possibile ridurli con l’introduzione di procedure ben definite. Tuttavia è da tener presente che non sarà mai possibile eliminare del tutto l’errore umano. La sorveglianza in una trasfusione è di responsabilità dell’infermiere, e la conoscenza delle reazioni avverse risulta carente. Riconoscere una reazione avversa e agire in tempo è importante: basta pensare ad una reazione emolitica acuta da incompatibilità ABO per la quale bastano poche gocce per mettere a rischio la vita del paziente.
