In Europe, epilepsy is the most common disabling neurological disease. It is characterized by the repetition of unpredictable, sudden and often very short crises, which take very different forms. These crises have long surrounded the disease with an aura of mystery. Today, we know that its origin is cerebral and linked to abnormal discharges within the neural networks that the electroencephalogram can record. In this thesis we will try to shed light on this disease, studying abnormal electrical discharges through electroencephalographic signals. We will focus on signal preprocessing methods, which are essential to be able to reliably analyze the paths and to highlight any morphological anomalies of the trace. Epilepsy is a pathology of the Central Nervous System, formed by the encephalon and the spinal cord; one of the components of the encephalon is the telencephalon, which divides into lobes of the cerebral cortex. The neurons that populate these regions of the cortex, and that are at the origin of the abnormal electrical discharge, are the pyramidal neurons. The bioelectrical activity of pyramidal neurons is that which is recorded by the electrodes during an electroencephalographic examination. The electroencephalograph is the tool that is used to detect the electrical potentials of the brain as a function of time. Knowing the lobe of origin of the abnormal electrical discharge is important to define the type of epilepsy and also to know if the crisis has a focal onset, if it involves only one hemisphere of the brain, or a generalized onset, if the crisis involves both hemispheres symmetrically. Defining the type of onset helps to find the cause of epilepsy and consequently to find a cure. To study the electroencephalographic signals we used EEGlab, a matlab toolbox. Before analyzing the signals, we subjected them to a pre-processing phase, which serves to eliminate noise and artifacts contained in the EEG recordings that overlap the signal produced by the neuronal activity of the brain and mask it. Artifacts are due to eye movements, head movements, facial muscle activity and electrode shifts. Preprocessing is crucial because without it natural movements, such as a blink of an eye, could be confused by seizures. To filter the signal, we applied a Notch filter, to eliminate line noise, and low pass and high pass filters to be able to consider only the frequency band related to the brain rhythms; then we eliminated the artefactative components, such as eye movements or facial muscles, with the Independent Component Analysis (ICA) method. Finally, we analysed the filtered electroencephalographic signals. We have seen that the epileptic activity is characterized by pointed graphoelements of great amplitude and short duration, which stand out compared to the underlying activity, at the level of the electrodes of the lobe affected by the crisis. So we tried to identify the sites where the epileptic seizure begins and the type of onset, focal or generalized; in the patient taken in analysis, the crisis has a focal onset, in the right temporal lobe. We also tried to understand the role of each brain rhythm during an epileptic seizure and observed that slow rhythms, such as theta and delta waves, are present at the onset of the crisis, while fast, gamma, rhythms appear during the crisis and persist even after the end of the crisis. This study highlighted the importance of automatic crisis detection systems and, above all, of pre-processing. without which the results of the analysis of an electroencephalographic signal cannot be considered reliable.

In Europa, l'epilessia è la malattia neurologica invalidante più comune. È caratterizzata dal ripetersi di crisi imprevedibili, improvvise e spesso molto brevi, che prendono forme molto diverse. Ecco perché dobbiamo parlare di epilessie e non di epilessia. Queste crisi hanno a lungo circondato la malattia con un alone di mistero. Oggi, sappiamo che la sua origine è cerebrale e legata a scariche anomale all'interno delle reti neurali che l'elettroencefalogramma può registrare. In questa tesi cercheremo di fare luce su questa malattia, studiando le scariche elettriche anomale attraverso i tracciati elettroencefalografici. Ci focalizzeremo sui metodi di preprocessamento del segnale, che sono fondamentali per poter analizzare in maniera affidabile i tracciati e per mettere in luce eventuali anomalie morfologiche del tracciato. L'epilessia è una patologia del Sistema Nervoso Centrale, formato dall'encefalo e dal midollo spinale; uno dei componenti dell'encefalo è il telencefalo, il quale si divide in lobi di corteccia cerebrale. I neuroni che popolano queste regioni della corteccia, e che sono all'origine della scarica elettrica anomala, sono i neuroni piramidali. L'attività bioelettrica dei neuroni piramidali è quella che viene registrata dagli elettrodi durante un esame elettroencefalografico. L'elettroencefalografo è lo strumento che viene utilizzato per rilevare i potenziali elettrici del cervello in funzione del tempo. Conoscere il lobo di origine della scarica elettrica anomala è importante per definire il tipo di epilessia e anche per sapere se la crisi sia ad esordio focale, se coinvolge solo un emisfero del cervello, o ad esordio generalizzato, se la crisi coinvolge entrambi gli emisferi in maniera simmetrica. Definire il tipo di esordio aiuta a trovare la causa dell'epilessia e di conseguenza a trovare una cura. Per studiare i tracciati elettroencefalografici abbiamo utilizzato EEGlab, un toolbox di Matlab. Prima di analizzare i segnali, li abbiamo sottoposti a una fase di pre-processamento, che serve a eliminare rumore e artefatti contenuti nelle registrazioni EEG che si sovrappongono al segnale prodotto dall’attività neuronale del cervello e la mascherano. Gli artefatti possono essere dovuti a movimenti oculari, movimenti della testa, attività muscolare del viso e spostamenti degli elettrodi. Il pre-processamento è fondamentale perché senza di esso movimenti naturali, come un battito di ciglia, potrebbero essere confusi per crisi epilettiche. Inizialmente, per filtrare il segnale, abbiamo applicato un filtro di Notch, per eliminare il rumore di linea, e dei filtri passa basso e passa alto per poter considerare solo la banda di frequenze relativa ai ritmi cerebrali; poi abbiamo eliminato le componenti artefattive, come i movimenti oculari o dei muscoli facciali, con il metodo di Analisi delle Componenti Indipendenti (ICA). Infine, abbiamo analizzato i segnali elettroencefalografici filtrati. Abbiamo visto che l'attività epilettica è caratterizzata da grafoelementi puntuti di grande ampiezza e breve durata, che spiccano rispetto all'attività di fondo, a livello degli elettrodi del lobo interessato dalla crisi. Quindi abbiamo cercato di individuare i siti in cui ha inizio la crisi epilettica e la tipologia di esordio, focale o generalizzato; nel paziente preso in analisi, la crisi è ad esordio focale, nel lobo temporale destro. Inoltre abbiamo cercato di capire il ruolo di ogni ritmo cerebrale durante una crisi epilettica e abbiamo osservato che i ritmi lenti, come le onde teta e delta, sono presenti all'esordio della crisi, mentre i ritmi veloci, gamma, compaiono durante la crisi e persistono anche dopo la fine della crisi. Da questo studio è emersa l'importanza dei sistemi automatici di rilevamento delle crisi e soprattutto del pre-processamento, senza il quale i risultati dell'analisi di un tracciato elettroencefalografico non possono essere considerati affidabili.

Preprocessing and interpretation of the electroencephalographic signal of subjects with epilepsy
