Rationale: Responsiveness is one of the three pillars of any health system, along with improved health and fair funding. Unlike the latter, however, it is often overlooked in both programming and monitoring, as it is difficult to measure. Responsiveness, in fact, is the experience of the people who use the health system: the respect with which they are treated, the communication they receive, their involvement in decisions, the pleasantness of the structures in which they are assisted, and much more. While it is important to verify the responsiveness of all users, the opportunity to investigate it among those who participate in cancer screening programs appears precious. These programs, in fact, carry out more less invasive tests on many healthy individuals, in order to improve the prognosis of those who are found to have a tumor at an early stage. Mammography screening, in particular, is more controversial. more and more complete evaluation. Objective of the study: Investigate Responsiveness within mammography screening programs. Materials and methods: Questionnaire, produced by re-adapting the "WHO Survey on Health" by the World Health Organization, administered to women aged between 50 and 69 who adhere to mammography screening for the prevention of breast cancer. Results: From the results obtained it can be deduced that overall the Responsiveness was evaluated with good scores. In particular, this result depends on some factors that have found higher scores. One of the first is the privacy with which women's information has been treated. A second factor is the respect with which the operators approached users, in particular that of the staff of the screening secretariat. Another factor that scored quite high was the timeliness with which the women got in touch with the screening secretariat, via email address or toll-free number, in order to provide them with appointments at the more suitable locations and less distant from the area of ​​residence (in fact, as can be seen from Table 1, the travel times to reach the hospital are less than 20 minutes for most women). A final factor that allowed Responsiveness to have high scores is that the screening services communicated with the users; starting from the invitation sent to women who were between the ages of 50 and 69 where the communication was defined for the most part perfectly clear; up to the relationship established between the operators and the women who wished to make an appointment to have a mammogram. As for the spaces and the wait inside the hospital, most of the ladies considered it good or more than good. Conclusions: Generally the Responsiveness within the screening services was evaluated with a score of 8, taking into consideration a scale from 1 to 10. The questionnaire was administered only to the women of Fabriano and Jesi, to have a more representative score the questionnaire should be administered to several people (at least 500 women) from different areas and then maybe even throughout Italy.

Razionale: La Responsiveness è uno dei tre pilastri di ogni sistema sanitario, insieme al miglioramento della salute e all’equo finanziamento. Diversamente da questi ultimi, però, è spesso tralasciata sia nella programmazione che nel monitoraggio, poiché è difficile misurarla. La Responsiveness, infatti, è l’esperienza delle persone che usano il sistema sanitario: il rispetto con cui sono trattate, la comunicazione che ricevono, il loro coinvolgimento nelle decisioni, la piacevolezza delle strutture in cui sono assistite, e molto di più. Mentre è importante verificare la Responsiveness di tutta l’utenza, appare preziosa l’opportunità di indagarla tra chi partecipa ai programmi di screening oncologico. Tali programmi, infatti, effettuano test più o meno invasivi su moltissimi individui sani, al fine di migliorare la prognosi di quella minoranza in cui viene riscontrato un tumore in fase precoce. Lo screening mammografico, in particolare, è il più controverso (le Raccomandazioni Europee del 2019 indicano minimi vantaggi nelle popolazioni sottoposte allo screening), e conoscere la Responsiveness di tale servizio tra le donne sane che vi si sottopongono regolarmente potrebbe fornire elementi utili ad una sua valutazione sempre più completa. Obiettivo dello studio: Indagare la Responsiveness all’interno dei programmi di screening mammografico. Materiali e metodi: Questionario, prodotto riadattando il "WHO Survey on Salute” dall’Organizzazione Mondiale della Sanità, somministrato alle donne di età compresa tra i 50 e i 69 anni che aderiscono allo screening mammografico per la prevenzione del tumore al seno. Risultati: Dai risultati ottenuti si può dedurre che nel complesso la Responsiveness è stata valutata con buoni punteggi. In particolare, questo risultato dipende da alcuni fattori che hanno riscontrato punteggi più alti. Uno dei primi è la privacy con cui le informazioni delle donne sono state trattate. Un secondo fattore è il rispetto con cui gli operatori si sono approcciati all’utenza in particolare quello avuto dal personale della segreteria degli screening. Un altro fattore che ha avuto un punteggio piuttosto alto è stata la tempestività con cui le donne si sono messe in contatto con la segreteria degli screening, tramite l’indirizzo e-mail o il numero verde, al fine di fornire loro degli appuntamenti presso le sedi più adatte e meno distanti dalla zona di residenza (di fatto come si può vedere dalla Tabella 1 per la maggior parte delle donne i tempi di viaggio per raggiungere l’ospedale sono inferiori a 20 minuti). Un ultimo fattore che ha permesso alla Responsiveness di avere dei punteggi alti è la comunicazione che i servizi di screening hanno adottato con l’utenza; a partire dall’invito spedito alle donne che avevano un’età compresa tra 50 e 69 anni dove la comunicazione è stata definita per lo più perfettamente chiara; fino ad arrivare alla relazione instaurata tra gli operatori e le donne che desideravano prendere un appuntamento per fare la mammografia. Per quanto riguarda gli spazi e l’attesa all’interno dell’ospedale la maggior parte delle signore l’hanno reputata buona o più che buona. Conclusioni: Generalmente la Responsiveness all’interno dei servizi di screening è stata valutata con un punteggio di 8, prendendo in considerazione una scala da 1 a 10. Il questionario è stato somministrato solo alle donne di Fabriano e Jesi, per avere un punteggio più rappresentativo bisognerebbe somministrare il questionario a più persone (minimo 500 donne) di diverse zone per poi magari estendere questo studio anche in tutta Italia.
