The subject of this thesis is to study the outcomes of intubation in patients who have suffered trauma; In fact, in these departments, I have often witnessed this particularly complex and delicate manoeuvre and that is why the desire was born in me to know whether this technique actually leads to an improvement in the clinical condition of the caregiver. However, the endotracheal intubation of the preosped patient is an invasive manoeuvre that is not without complications; in fact, it is considered by some perpetrators to be harmful, dangerous, increases mortality and results in an unjustified waste of precious time. The study of other articles shows that 95% of intubated people are more likely to survive if they are intubated in the emergency room, then inside the hospital; while people intubated at the scene, are less likely to survive due to a difficulty in performing the maneuver or a misplaced endotracheal tube. Thus, there is a discrepancy between the various articles of the literature and evaluating several works it would seem that intubation is not always effective for the survival of the patient and therefore a maneuver to be carefully evaluated if and when to carry out. The purpose of this thesis, based on the literature collected and analyzed, is to go and see what are the results of intubation in patients who have suffered trauma and who are transported within the hospital reality of the Trauma Center of Torrette.
Oggetto di questa tesi è quello di studiare gli esiti dell’intubazione nei pazienti che hanno subito un trauma; infatti, in questi reparti, ho assistito spesso a questa manovra particolarmente complessa e delicata ed è per questo motivo che è nata in me la voglia di sapere se questa tecnica porti effettivamente ad un miglioramento della condizione clinica dell’assistito. L’intubazione endotracheale del paziente traumatizzato a livello preospedaliero rappresenta però una manovra invasiva non priva di complicanze; infatti è considerata da alcuni autori deleteria, pericolosa, aumenta la mortalità e comporta un’ingiustificata perdita di tempo prezioso. Dallo studio di altri articoli si evince che il 95% delle persone intubate hanno più probabilità di sopravvivenza se vengono intubate in sala emergenza, quindi all’interno dell’ospedale; mentre le persone intubate sulla scena, hanno meno probabilità di sopravvivenza a causa di una difficoltà nell’effettuare la manovra o di un mal posizionamento del tubo endotracheale. Esiste, così, una discrepanza tra i vari articoli della letteratura e valutando diversi lavori sembrerebbe che l’intubazione non sia sempre efficace per la sopravvivenza del paziente e quindi una manovra da valutare attentamente se e quando effettuare. Lo scopo di questa tesi, sulla base della letteratura raccolta ed analizzata, è quello di andare a vedere quali sono i risultati dell’intubazione nei pazienti che hanno subito un trauma e che vengono trasportati all’interno della realtà ospedaliera del Trauma Center di Torrette.
