ABSTRACT: The experience of Burnout in the caregivers of Alzheimer’s patients: critical review of literature. BACKGROUND: "Burnout" as a phenomenon was first described in 1974 by psychologist Herbert Freudenberger. It is defined as a condition of physical and mental exhaustion related to care activities and arises as a consequence of chronic exposure to interpersonal stressors. The World Health Organization, in the tenth revision of its International Classification of Diseases, called it "a state of vital exhaustion". OBJECTIVE: The primary objective of this text is to describe the emotional impact on the emotional and motivational sphere of caregivers. The secondary objective is to provide the right tools to overcome the frustration in the depersonalization phase, in the feeling of inadequacy for the task that the caregiver is carrying out. MATERIALS AND METHODS: A critical review of the literature was conducted on the major MEDLINE databases through PUB MED, CHINAL, COCHRANE LIBRARY and SCOPUS. To narrow the search, we used MeSH terms from the Thesaurus library crossed with free terms such as "BURNOUT, CARGIVER, EMOTIONAL EXHAUSTION". 31 articles were found, 7 of which were deleted because they were irrelevant and did not meet the search criteria. Only 24 articles were considered, including RCTs, meta-analyzes, systematic and scoping reviews in English published in the last 10 years. RESULTS: The results obtained from the analysis of the articles taken into consideration are quite complementary to each other. Relevant is the fact that Alzheimer's disease has been found to be the most common dementia worldwide. Furthermore, it emerged that Burnout is becoming a real epidemic: 50% of the caregivers taken into consideration and involved in the studies suffered from Burnout syndrome, with a prevalence mainly due to the symptom of emotional exhaustion. The factors that determine the state of emotional exhaustion are probably exogenous as evidenced by the number of studies conducted. The most relevant data are the prolonged exposure to stress factors related to care, the absence or reduced social and health support, critical socio-economic and cultural contexts. The respite-care interventions have demonstrated undoubted effectiveness in terms of health of the caregiver and the patient, who have improved in monitoring through the scales used, reducing the effects of depression. DISCUSSION: It is necessary that the system, especially the health system, both from a territorial and national point of view, does not underestimate this problem, thus strengthening the formal and informal social networks, in such a way as to guarantee protection and support for both parties. It may also be necessary to implement the literature regarding the topic of patient prevention and home management with innovative educational techniques. Ensuring adequate care for Alzheimer's patients also means protecting those who take care of them; "Taking care of those who care" is a civic and moral duty. CONCLUSIONS: It is therefore clear that for Alzheimer's disease there will be an exponential increase in incidence and prevalence numbers of about 4 times by 2050 in the elderly over 65 years of age. The main needs that have been perceived by the caregiver are, essentially, the management of their family member with dementia and the management of their health. The incidence of Burnout is also closely related to the cultural level of caregivers, in fact those with a higher cultural level can undertake early respite care paths, on the contrary those who do not have a satisfactory cultural level or language problems find it very difficult to find the right paths to help.

BACKGROUND: Il "Burnout" come fenomeno fu descritto per la prima volta nel 1974 dallo psicologo Herbert Freudenberger. È definita come una condizione di esaurimento fisico e mentale relativa alle attività di assistenza e nasce come conseguenza dell'esposizione cronica a fattori di stress interpersonali. L'Organizzazione mondiale della sanità, nella decima revisione della sua Classificazione internazionale delle malattie, lo ha definito "uno stato di esaurimento vitale". OBIETTIVO: L’obiettivo primario di questo elaborato è descrivere l’impatto emotivo sulla sfera affettiva e motivazionale dei caregiver. L’obiettivo secondario è fornire i giusti strumenti per superare la frustrazione nella fase di depersonalizzazione, nella sensazione di inadeguatezza al compito che sta svolgendo il caregiver. MATERIALI E METODI: È stata condotta una revisione critica della letteratura, sui maggiori data base MEDLINE attraverso PUB MED, CHINAL, COCHRANE LIBRARY e SCOPUS. Per restringere la ricerca abbiamo utilizzato termini MeSH dalla biblioteca Thesaurus incrociati con termini liberi come “BURNOUT, CARGIVER, EMOTIONAL EXHAUSTION”. Sono stati reperiti 31 articoli di cui 7 eliminati perché non pertinenti e non rispondevano ai criteri di ricerca. Sono stati presi in considerazione solo 24 articoli di cui, RCT, metanalisi, systematic and scoping reviews in lingua inglese pubblicati negli ultimi 10 anni. RISULTATI: I risultati ottenuti dall’analisi degli articoli presi in considerazione sono abbastanza complementari tra di loro. Rilevante è il fatto che la malattia di Alzheimer è risultata la più comune demenza in tutto il mondo. Inoltre, è emerso che il burnout sta diventando una vera e propria epidemia: il 50% dei caregivers presi in considerazione e coinvolti negli studi, soffrivano della sindrome da Burnout, con una prevalenza soprattutto per il sintomo dell’esaurimento emotivo. I fattori che determinano lo stato di esaurimento emotivo sono verosimilmente esogeni testimoniato dai numeri studi condotti. I dati più rilevanti sono l’esposizione prolungata ai fattori di stress correlati all'assistenza, l’assenza o ridotto supporto sociosanitario, contesti critici a livello socioeconomico e culturale. Gli interventi di respite-care hanno dimostrato un’indubbia efficacia in termini di salute del caregiver e del paziente, i quali sono migliorati nel monitoraggio attraverso le scale utilizzate riducendo gli effetti della depressione. DISCUSSIONE: È necessario che il sistema, soprattutto sanitario, sia da un punto di vista territoriale che nazionale, non sottovaluti questo problema, rafforzando, quindi, le reti sociali formali e informali, in modo tale da garantire protezione e supporto per entrambe le parti. Potrebbe anche essere necessario che si implementi la letteratura per quanto riguarda il tema della prevenzione e gestione domiciliare del paziente con tecniche educative innovative. Garantire un’assistenza adeguata ai pazienti affetti da Alzheimer rappresenta anche tutelare chi si prende cura di questi ultimi; “curare chi cura” è un dovere civico e morale. CONCLUSIONI: Si evince, quindi, che per la malattia di Alzheimer ci sarà un aumento esponenziale dei numeri di incidenza e prevalenza di circa 4 volte entro il 2050 negli anziani con età maggiore di 65 anni. I bisogni principali che sono stati percepiti da parte del caregiver sono, essenzialmente, la gestione del proprio familiare con demenza e la gestione della propria salute. L’incidenza del Burnout è strettamente correlata anche al livello culturale dei caregiver, infatti chi ha un livello culturale più alto riesce a intraprendere percorsi precoci di respite care, al contrario chi non ha un livello culturale soddisfacente o problemi di lingua fa molta fatica a trovare i giusti percorsi di aiuto.
