BACKGROUND: Patellar tendinopathy (patellar tendinopathy) is a clinical condition characterized by the presence of localized pain in the area of insertion of the patellar tendon at the lower pole of the patella. The disease is related to increased stress on the extensive mechanism of the knee, particularly during energy storage and release activities. To date, patellar tendinopathy has several aspects not yet clarified by scientific literature, in fact there is no uniform consensus for a gold-standard treatment. OBJECTIVES: The aim of this review is to seek, among the various literature studies, the best scientific evidence about effective methods of conservative treatment to reduce pain and increase function in subjects with patellar tendinopathy. These two aspects are measured in the studies audited with the respective outcome measures. The research focused on physiotherapy research. RESEARCH METHODOLOGIES: the research was carried out through the Pubmed database. After a restriction of the inclusion criteria, 8 different studies were selected, mostly systematic revisions that proposed different conservative strategies, for the treatment of subjects with patellar tendinopathy. RESULTS: Of the 767 articles found in the databases, 8 articles were finally selected to conduct this review of the literature. The studies included are mostly systematic reviews (5), randomized controlled studies (3). As outcome measurements, scales were often used for pain assessment, provocative tests to diagnose patellar tendinopathy, and various clinical tests to assess the impact of treatment. CONCLUSIONS: Patellar tendinopathy is a pathology, often chronic, that presently presents various aspects not yet clarified by the scientific literature. For rehabilitation treatment there are no real validated protocols to be implemented. Despite the heterogeneity of the studies, physiotherapy treatments (e.g. eccentric, HSR, load management) as well as passive treatments such as shock wave therapy (ESWT) or platelet-enriched plasma (PRP) have been found to be effective.
BACKGROUND: La tendinopatia rotulea (o patellar tendinopathy) è una condizione clinica caratterizzata dalla presenza di dolore localizzato nella area di inserzione del tendine rotuleo a livello del polo inferiore della rotula. La patologia è correlata ad un aumento dello stress sul meccanismo estensorio del ginocchio, in particolare durante le attività di immagazzinamento e rilascio di energia. Ad oggi la tendinopatia rotulea presenta vari aspetti non ancora chiariti dalla letteratura scientifica, infatti non esiste un consenso uniforme nei confronti di un trattamento gold-standard. OBIETTIVI: L’obiettivo di questa revisione è ricercare, tra i vari studi della letteratura, le migliori evidenze scientifiche presenti riguardo dei metodi efficaci di trattamento conservativo per diminuire il dolore ed incrementare la funzione in soggetti affetti da tendinopatia rotulea. Questi due aspetti vengono misurati negli studi sottoposti a revisione con le rispettive misure di outcome. La ricerca si è concentrata su ricerche fisioterapiche. METODOLOGIE DI RICERCA: la ricerca è stata effettuata attraverso il database PubMed. Dopo una restrizione dei criteri di inclusione, sono stati selezionati 8 diversi studi, per la maggior parte revisioni sistematiche che proponevano diverse strategie conservative, per il trattamento di soggetti affetti da tendinopatia rotulea. RISULTATI: Dei 767 articoli trovati nelle banche dati, sono stati selezionati infine 8 articoli utili per condurre questa revisione della letteratura. Gli studi inclusi sono per la maggior parte revisioni sistematiche (5), studi randomizzati controllati (3). Come misure di outcome, sono state spesso utilizzate scale per la valutazione del dolore, test provocativi per diagnosticare la tendinopatia rotulea e vari test clinici per valutare l’impatto del trattamento. CONCLUSIONI: La tendinopatia rotulea è una patologia, spesso cronica, che presenta attualmente, vari aspetti non ancora chiariti dalla letteratura scientifica. Per il trattamento riabilitativo non esistono veri e propri protocolli validati da attuare. Nonostante l’eterogeneità degli studi si sono rilevati di buona efficacia sia i trattamenti fisioterapici (es. eccentrico, HSR, Load management) sia quelli passivi come la terapia con onde d’urto (ESWT) o quella con plasma arricchito di piastrine (PRP).
