Introduction In a constantly evolving healthcare context, the guarantee of increasingly high-level assistance and care becomes central to the activity carried out by a healthcare company. The ISO 9001:2015 standard is the international standard used to certify Quality Management Systems, adopted for this by AST Macerata, in which the study was conducted, to certify the OUs, increase user satisfaction and reduce risks deriving from internal processes. Goal The following objectives were set at the basis of the study: - Understand the indications provided by the ISO 9001 standard with a view to the planning, management and evaluation of care services; - Contextualize the standard in the reference healthcare company; - Evaluate the results obtained over the years in terms of continuous improvement. Materials and methods To contextualize the general information obtained from the analysis of the ISO standard and the clinical audit structure defined by the Ministry of Health in 2011, it was deemed appropriate to relate these concepts to the reality of AST Macerata, evaluating the documents developed, observing the potential of the certification of QMS in a real context and evaluating the improvements it entails. For this purpose, collaboration was made with the Health Professions Service of AST Macerata and with the O.U. Oncology, to better analyze the process that leads to certification and its maintenance over time. The study was conducted in the year 2023. Results The internal and external audits carried out were compared, to evaluate the actions taken following the reports made. In the audits examined, no major Non-Conformities were found, a sign of good application of the indications contained in the ISO standard by the OUs. Only a few minor non-conformities were reported, resolved in all cases. For the Observations made, corrective actions capable of bringing improvements to the QMS and therefore greater safety for the interested parties were quickly implemented. Discussion The weak points found in the execution of the audits have been grouped in a comparative table and divided into Observations and Non-Conformities, inserting the corrective or preventive actions implemented in a column alongside. The results were then further rationalized with specific indicators. The Non-Conformities and Observations identified in the study have progressively decreased over time. This is interpreted as a clear sign of continuous improvement aimed at increasing the safety and quality of the services provided. We note the total absence of serious non-conformities, indicating excellent application of the indications of the ISO standard since the beginning of the certification process. Conclusions The study started from the analysis of the ISO 9001 standard and the structure of the clinical audit, compared to the local reality of the AST Macerata and observing the results achieved in terms of risk prevention and continuous improvement of the quality of the services provided. The analysis and cross-comparison of the audit reports carried out over the years in the UUOOs under study highlight a trend of continuous improvement in the quality of the processes and services provided, the central point of the certification process.
Introduzione In un contesto sanitario in continua evoluzione, la garanzia di assistenza e cure sempre di maggior livello diventa centrale nell’attività svolta da un’azienda sanitaria. La norma ISO 9001:2015 è lo standard internazionale utilizzato per certificare i Sistemi di Gestione per la Qualità, adottata per questo dall’AST Macerata, nella quale è stato condotto lo studio, per certificare le UUOO, aumentare la soddisfazione dell’utente e ridurre i rischi derivanti dai processi interni. Obiettivi Alla base dello studio sono stati posti i seguenti obiettivi: - Comprendere le indicazioni fornite dalla norma ISO 9001 nell’ottica della programmazione, gestione e valutazione dei servizi assistenziali; - Contestualizzare la norma nell’azienda sanitaria di riferimento; - Valutare i risultati ottenuti negli anni in termini di miglioramento continuo. Materiali e metodi Per contestualizzare le informazioni generali ottenute dall’analisi della norma ISO e della struttura dell’audit clinico definita dal Ministero della Salute nel 2011, si è ritenuto opportuno rapportare tali concetti alla realtà dell'AST Macerata, valutando i documenti sviluppati, osservando le potenzialità della certificazione dei SGQ in un contesto reale e valutando i miglioramenti che essa comporta. A tale scopo si è usufruito della collaborazione con il Servizio delle Professioni Sanitarie dell’AST Macerata e con l’U.O. Oncologia, per meglio analizzare l’iter che porta alla certificazione e al suo mantenimento nel tempo. Lo studio è stato condotto nell’anno 2023. Risultati Si sono confrontati gli audit interni ed esterni effettuati, per valutare le azioni intraprese a seguito delle segnalazioni effettuate. Negli audit visionati non si sono riscontrate Non Conformità importanti, segno di una buona applicazione delle indicazioni contenute nella norma ISO da parte delle UUOO. Sono state riportate solo alcune Non Conformità lievi, risolte nella totalità dei casi. Per le Osservazioni effettuate sono state rapidamente implementate azioni correttive in grado di apportare miglioramenti al SGQ e pertanto maggiore sicurezza per le parti interessate. Discussione I punti di debolezza riscontrati nell’esecuzione degli audit, sono stati raggruppati in una tabella comparativa e suddivisi in Osservazioni e in Non Conformità, inserendo in una colonna al fianco le azioni correttive o preventive messe in atto. I risultati poi sono stati ulteriormente razionalizzati con appositi indicatori. Le Non Conformità e le Osservazioni individuate nello studio, sono progressivamente diminuite nel tempo. Si interpreta questo come un chiaro segno di miglioramento continuo finalizzato ad aumentare sempre di più la sicurezza e la qualità dei servizi erogati. Si segnala la totale assenza di Non Conformità gravi, indice di un’ottima applicazione delle indicazioni della norma ISO sin dall’inizio del percorso di Certificazione. Conclusioni Lo studio si è avviato dall’analisi della norma ISO 9001 e della struttura dell’audit clinico, rapportati alla realtà locale dell'AST Macerata e osservando i risultati raggiunti in termini di prevenzione dei rischi e di miglioramento continuo della qualità dei servizi erogati. L’analisi ed il confronto incrociato dei report degli audit effettuati negli anni nelle UUOO oggetto di studio, evidenziano un trend di miglioramento continuo nella qualità dei processi e dei servizi erogati, punto centrale del processo di certificazione.
