Abstract This thesis aims to improve the quality, consistency, and reliability of information flows in the Home Care Services of AST Macerata through a data-driven approach focused on strengthening healthcare governance and promoting a solid data culture. In a national context marked by operational disparities, service fragmentation, and delays in territorial innovation, the enhancement of the information heritage emerges as a strategic lever to increase the effectiveness, equity, and sustainability of health and socio-health services. The study is structured around four main pillars: analysis of critical issues in data collection and management; definition of standardized operational procedures; design and implementation of replicable and sustainable improvement actions; and promotion of a data culture among professionals through targeted training programs. The research, conducted as a retrospective observational study with a qualitative-quantitative approach, analyzed data from 2023–2024 regarding the number of patients assisted, care plans (PIC), home visits, and the volume and type of services delivered across the three Districts of AST Macerata. Data processing, carried out through queries, tables, and pivot charts, revealed substantial discrepancies and inconsistencies among Districts, hindering effective territorial planning. The process required thorough data cleaning, validation, and analysis, uncovering technical, organizational, and professional challenges, further exacerbated by exceptional events such as the 2023 regional data loss. These findings guided the development of an improvement plan based on shared and sustainable procedures, supported by training initiatives aimed at strengthening data literacy and harmonizing operational practices. In the current setting marked by increasingly complex clinical, social, economic, and cultural vulnerabilities integrated management between healthcare and social professionals demands reliable, rigorously analyzed, and shared data. Only a conscious and strategic use of information resources can drive effective organizational decision-making, reduce waste, and enhance the primacy of care over mere service provision. The project concludes with the establishment of a continuous monitoring system to support flexible and efficient decision-making processes, aligned with the SSN’s mission of proximity care, and with an operational horizon extending through 2026.
Abstract Questo elaborato di tesi si propone di migliorare la qualità, l’uniformità e l'affidabilità dei flussi informativi nei Servizi di Assistenza Domiciliare dell’AST Macerata, attraverso un approccio data-driven orientato al rafforzamento della governance sanitaria e alla diffusione di una solida cultura del dato. In un contesto Nazionale segnato da disomogeneità operative, frammentazione dei servizi e ritardi nell’innovazione territoriale, la valorizzazione del patrimonio informativo rappresenta una leva strategica per aumentare efficacia, equità e sostenibilità nei servizi sanitari e sociosanitari. Lo studio si sviluppa lungo quattro direttrici principali: analisi delle criticità nella raccolta e gestione dei dati; definizione di modalità operative standardizzate; progettazione e attuazione di interventi migliorativi replicabili e sostenibili; promozione di una cultura del dato tra i professionisti tramite un percorso formativo mirato. La ricerca, di tipo osservazionale retrospettivo con approccio quali-quantitativo, ha analizzato i dati del biennio 2023–2024 relativi a numero di assistiti, prese in carico (PIC), prestazioni (per tipologia e volume) e accessi domiciliari nei tre Distretti dell’AST Macerata. L’elaborazione dei dati è avvenuta tramite query, tabelle e pivot, e ha messo in evidenza una profonda disomogeneità tra i Distretti, con dati non conformi che hanno ostacolato l’efficacia della programmazione territoriale. L’intero processo ha richiesto un attento lavoro di pulizia, validazione e analisi, evidenziando criticità tecniche, organizzative e professionali, ulteriormente aggravate dalla perdita del patrimonio informativo regionale nel 2023. Le evidenze emerse hanno portato alla costruzione di un piano di miglioramento basato sull’adozione di procedure condivise e sostenibili, affiancato da attività formative volte a rafforzare la consapevolezza e le competenze legate alla data literacy. Nel contesto attuale, segnato da fragilità sempre più complesse di natura clinica, sociale, economica e culturale, la gestione integrata tra professionisti sanitari e sociali richiede un sistema informativo affidabile, rigoroso e condiviso. Solo un utilizzo consapevole del patrimonio informativo consente di orientare le scelte organizzative verso interventi realmente efficaci, evitando sprechi e valorizzando la centralità della cura rispetto alla mera prestazione. Il progetto si conclude con la definizione di un sistema di monitoraggio continuo, volto a supportare decisioni efficaci e flessibili, in linea con la missione di prossimità del SSN, con un orizzonte operativo esteso fino al 2026.
