This thesis analyzes an innovative non-invasive prenatal testing (NIPT) approach for the diagnosis of thalassemia, based on the analysis of cell-free fetal DNA (cffDNA) circulating in maternal plasma and on parental haplotype inference. The aim of the study is to improve fetal genotype determination in pregnancies at risk of monogenic disorders, overcoming the limitations of conventional NIPT protocols. To achieve this, the proposed method integrates cffDNA enrichment, target region capture, next-generation sequencing (NGS), and a dedicated bioinformatics pipeline based on haplotype reconstruction. The results demonstrated high diagnostic accuracy, with a 98.16% success rate and 100% concordance with invasive prenatal diagnosis. Overall, the study highlights the potential of this approach as a promising strategy for the non-invasive prenatal diagnosis of thalassemia and, potentially, other monogenic disorders.
Il presente lavoro analizza un approccio innovativo di test prenatale non invasivo (NIPT) per la diagnosi della talassemia, basato sull’analisi del DNA fetale libero circolante (cffDNA) e sull’inferenza degli aplotipi parentali. L’obiettivo dello studio è migliorare la determinazione del genotipo fetale nelle gravidanze a rischio di malattie monogeniche, superando i limiti dei protocolli convenzionali. A tal fine, è stato sviluppato un protocollo che integra l’arricchimento del cffDNA, la cattura delle regioni target, il sequenziamento di nuova generazione (NGS) e un’analisi bioinformatica basata sulla ricostruzione degli aplotipi. I risultati dimostrano un’elevata accuratezza diagnostica, con un tasso di successo del 98,16% e una concordanza del 100% rispetto alla diagnosi prenatale invasiva. Nel complesso, lo studio evidenzia come questo approccio rappresenti una promettente strategia per la diagnosi prenatale non invasiva della talassemia e, potenzialmente, di altre malattie monogeniche.
