This thesis examines Paediatric Palliative Care as a clinical, relational and organisational field in which professional competencies, caregiver participation and continuity of care contribute to the development of a genuinely child- and family-centred approach. After reconstructing the international and regulatory development of paediatric palliative care, the thesis explores the meaning of the caring relationship, the role of professionals and caregivers, shared decision-making processes, and forms of empowerment that support care throughout the course of an incurable illness. The empirical section analyses the Regional Referral Centre for Pain Therapy and Paediatric Palliative Care of the Marche Region, considering it as a case of organisational innovation. Through the testimonies of healthcare professionals and caregivers, the research investigates the meaning attributed to the Centre, its coordination functions and its role in ensuring continuity of care, highlighting available resources, critical issues and the conditions required to consolidate the care network. The findings show that innovation does not consist merely in establishing a service, but in the capacity to integrate regulations, professional competencies, shared decision-making and relationships. From this perspective, caring means recognising the vulnerability of the other person and safeguarding their dignity, voice and humanity throughout the entire care pathway.
