This project proposes a strategy for the organisational optimisation of the in-hospital diagnostic pathway for geriatric inpatients at the INRCA hospital in Ancona. An analysis of workflows in current clinical practice (the ‘As-Is’ model) has identified the handover phase between inpatient wards and the Diagnostic Imaging Department as an extremely critical organisational bottleneck; indeed, frequent issues were identified regarding patient identification, gaps in medical-legal documentation, inefficiencies in peripheral venous access, and communication failures regarding the patient’s clinical conditions. These are issues that expose the patient, healthcare professionals and the entire organisation to a high clinical risk. To mitigate these issues, by integrating the principles of Clinical Risk Management and Lean Healthcare, a standardised checklist has been designed for use during the patient preparation phase in the inpatient ward: conceived as an operational tool, it requires the sending ward to carry out a rigorous dichotomous verification of certain essential requirements prior to the patient’s transfer. The expected benefits include a reduction in adverse events, improved patient safety, and reduced waiting times and lengths of stay. The adoption of this checklist, with a focus on organisational improvement, makes a transition from a protection-based approach to one centred on the prevention of harm, promoting a healthcare culture characterised by increased safety and providing a flexible, cross-cutting tool that can be applied across a wide range of care settings.

Abstract
The adoption of this checklist, with a focus on organisational improvement, makes a transition from a protection-based approach to one centred on the prevention of harm, promoting a healthcare culture characterised by increased safety and providing a flexible, cross-cutting tool that can be applied across a wide range of care settings.| File | Dimensione | Formato | |
