BACKGROUND: In Italy, the nursing career develops mainly horizontally: the acquisition of specialization masters, which increase the specific skills of the professional, does not in the majority of cases sanction an advancement of status in terms of remuneration or career. The national collective labor agreement for the health sector of 2018 introduces the figure of the specialist nurse or the professional with a first level master's degree and advanced competence in a specific clinical setting. OBJECTIVES: To analyze the occupational realities and the respective "advanced" roles of specialist nurses by assessing the occupational areas and roles held in the health care hospital Ospedali Riuniti Marche Nord, as well as the implications of the aforementioned aspects in terms of gratification and personal expectations. MATERIALS AND METHODS: All the nurses of the Ospedali Riuniti Marche Nord were involved with an open-ended contract operating in the company between 2019 and 2020. Data were obtained from the personal curricula of the employees and through the administration of a questionnaire. RESULTS: From a first analysis of the data obtained from the company mapping, it emerged that one nurse out of four of the hospital hospital Ospedali Riuniti Marche Nord has a first level master and therefore could be classified as a specialized professional. The questionnaire revealed the main motivations and areas of specialist interest on the part of the nurses but also the perception of the role itself by the professionals and colleagues. LIMITS: Monocentric study with low response rate. CONCLUSIONS: The reforming impact of the new specialists in the world of work still appears to be characterized by some shadows probably due to the regulatory complexity and bureaucratic rigidity of the systems of production of services which, still with difficulty, conceive job positions different from those traditionally carried out. The fact that emerges, in fact, is that there is no shortage of specialists but their activity is confused in basic care. It is therefore necessary that this new professional figure makes the elements of novelty it brings more clearly and precisely recognizable, specifying how, and why, to refer to these professionals rather than others.
BACKGROUND: In Italia, la carriera infermieristica si sviluppa prevalentemente in senso orizzontale: l’acquisizione di masters di specializzazione, che accrescono le competenze specifiche del professionista, non sancisce, nella maggioranza dei casi, un avanzamento di status in termini di retribuzione o carriera. Il contratto collettivo nazionale del lavoro per il comparto sanità del 2018 introduce la figura dell’infermiere specialista ovvero il professionista in possesso di master di primo livello e competenza avanzata in uno specifico ambito clinico. OBIETTIVI: Analizzare le realtà occupazionali e i rispettivi ruoli “avanzati” degli infermieri specialisti valutando gli ambiti occupazionali e i ruoli ricoperti nell’azienda sanitaria Ospedali Riuniti Marche Nord, nonché le implicazioni degli suddetti aspetti in termini di gratificazione e aspettative personali. MATERIALI E METODI: Sono stati coinvolti tutti gli infermieri degli Ospedali Riuniti Marche Nord con un contratto a tempo indeterminato operanti in azienda tra il 2019 e il 2020. Sono stati ottenuti i dati dai curricula personali dei dipendenti e attraverso la somministrazione di un questionario. RISULTATI: Da una prima analisi dei dati ottenuti dalla mappatura aziendale, è emerso che un infermiere su quattro dell’azienda ospedaliera Ospedali Riuniti Marche Nord possiede un master di primo livello e quindi potrebbe essere inquadrato come professionista specializzato. Dal questionario sono emerse le principali motivazioni e aree di interesse specialistico da parte degli infermieri ma anche la percezione del ruolo stesso da parte dei professionisti e dei colleghi. CONCLUSIONI: L’impatto riformatore dei nuovi specialisti nel mondo del lavoro appare caratterizzato ancora da qualche ombra probabilmente dovuta alla complessità normativa e alla rigidità burocratica dei sistemi di produzione di servizi che, ancora con difficoltà, concepiscono posizioni lavorative diverse da quelle tradizionalmente svolte. Il dato che emerge, infatti, è che non mancano gli specialisti ma la loro attività è confusa nell’assistenza di base. È, quindi, necessario che questa nuova figura professionale renda maggiormente evidenti, in modo preciso e riconoscibile, gli elementi di novità che porta, precisando come, e perché, far riferimento a questi professionisti piuttosto che ad altri.
