This thesis is a narrative review of the literature, in which the focus is on the definition, but above all on the treatment of phantom limb pain in the amputee. It is a very frequent disorder in subjects who undergo this type of surgery and involves symptoms with such a high level of painful intensity, as to limit the performance of normal daily life activities and cause a marked worsening of the quality of life of the subjects concerned. Very little is known about both the etiology of the disorder and the possible therapies; in fact, there are no guidelines that can be followed. Currently, the most accredited hypothesis regarding etiology is the cortical reorganization of the somato-sensory and motor cortices, associated with an alteration of the peripheral nervous system and spinal cord. As regards the treatment, on the other hand, various therapies have been proposed, of a surgical, pharmacological, cognitive-behavioral and physiotherapy type, but none of them have given scientifically relevant results to be included in the guidelines.
Questa tesi è una revisione narrativa della letteratura, in cui l’attenzione si concentra sulla definizione, ma soprattutto sul trattamento del dolore da arto fantasma nell’amputato. Si tratta di un disturbo molto frequente nei soggetti che subiscono questo tipo di intervento chirurgico e comporta sintomi con un livello talmente elevato di intensità dolorosa, da limitare lo svolgimento delle normali attività di vita quotidiana e provocare un netto peggioramento della qualità della vita dei soggetti interessati. Si sa ancora molto poco sia per quanto riguarda l’eziologia del disturbo, che le terapie possibili; non esistono infatti linee guida da poter seguire. Attualmente l’ipotesi più accreditata riguardante l’eziologia è la riorganizzazione corticale delle cortecce somato-sensoriale e motoria, a cui si associa un’alterazione del sistema nervoso periferico e del midollo spinale. Per quanto riguarda il trattamento invece sono state proposte diverse terapie, di tipo chirurgico, farmacologico, cognitivo-comportamentale e fisioterapico, ma nessuna di esse ha dato risultati scientificamente rilevanti per poter essere inserita nelle linee guida.
