BACKGROUND In literature cervicogenic headache is scarcely studied despite it afflicting a large percentage of population. Furthermore, the available studies therapeutic approach, although effective, doesn’t suggest a single treatment. Research of a real targeted therapy could bring forth better results for both relief and the reduction of the patients’ social and economic discomfort. PURPOSE OF THE RESEARCH The purpose of this research is the evaluation of manual therapy and therapeutic exercise as treatments for cervicogenic headache. MATERIALS AND METHODS Searches were carried out in the databases of PubMed and Cochrane Review. RCTs were selected that compared manual techniques or therapeutic exercise with each other and others evaluating the same treatments with respect to placebo or no intervention. RESULTS The studies analyse different types of manual therapeutic approach and the main outcomes regarding cervicogenic headaches originating from disfunction of the cervical spine and of the neck muscles. Overall pain intensity, frequency and duration undergo a short-term and long-term reduction following all the examined treatments. The cervical ROM measured in three studies also increased in a satisfying way thanks to the intervention on trigger points, UC-TSM and self-SNAG. Regarding performance on the cranio-cervical flexion muscle test, excellent results have been achieved with trigger point therapy and therapeutic exercise both with and without associated manipulative therapy. CONCLUSIONS The analysed studies demonstrate the effectiveness of conservative treatments regarding the most important outcomes considered in this review (pain intensity, cervical spine mobility, drugs reduction, subjective relief).

BACKGROUND In letteratura la cefalea cervicogenica è scarsamente studiata nonostante sia una problematica che affligge una discreta percentuale di popolazione. Inoltre negli studi reperibili l’approccio terapeutico, pur dimostrandosi efficace, non propone univocità di trattamenti. La ricerca di una reale terapia mirata potrebbe dare migliori risultati sia nel sollievo sia nel ridurre i disagi sociali ed economici dei pazienti. SCOPO DELLA RICERCA Lo scopo di questa ricerca è la valutazione della terapia manuale e dell’esercizio terapeutico come trattamenti per la cefalea cervicogenica. MATERIALI E METODI La ricerca è stata eseguita nei database PubMed e Cochrane Review. Sono stati prescelti RCT che confrontassero tecniche manuali o esercizio terapeutico tra loro e altri di valutazione degli stessi trattamenti rispetto a placebo o nessun intervento. RISULTATI I lavori analizzano tipi diversi di approccio terapeutico manuale e i principali outcome riguardanti le cefalee cervicogeniche originate da disfunzioni del rachide cervicale e della muscolatura del collo. In generale l’intensità, la frequenza e la durata del dolore si riducono sia a breve che a lungo termine a seguito di tutti i trattamenti presi in esame. Anche il ROM cervicale misurato in tre lavori è aumentato in modo soddisfacente con l’intervento sui trigger point, con UC-TSM e con il self-SNAG. Per quanto riguarda la performance muscolare dei flessori cranio-cervicali si hanno ottimi risultati con la manovra di pressione sui trigger point e con l’esercizio terapeutico da solo e in abbinamento alla terapia manipolativa. CONCLUSIONI Gli studi analizzati dimostrano l’efficacia dei trattamenti conservativi nei confronti dei più significativi outcome presi in considerazione (intensità del dolore, mobilità del rachide cervicale, riduzione dei farmaci, benessere soggettivo).
