In this study reference was made to a classification proposed in 1990 by a famous Danish professor, Gøsta Espring-Andersen, which subdivides social Europe into three Welfare Regimes (Liberal, Conservative, Social-Democratic), and the revision of these Regimes carried out by Maurizio Ferrera in 2006. Espring-Andersen manages to group and classify the social system of the European States, while Ferrera subsequently brings an important change to the classification in order to introduce the concept of the Fourth Europe social, or the addition of a new type of Welfare, called Mediterranean, which includes Italy, Spain, Portugal and Greece. It is precisely on the basis of this classification proposed by Ferrera that it was possible to develop the present study: Italy, Spain, Portugal and Greece are united by a very similar historical and social evolution that allows more easily to identify similarities and differences useful for a comparison between the NHS. In the first part of the thesis the historical and social evolution of each country is analyzed, from the end of the nineteenth century to the present day, paying particular attention to the health policies implemented in each historical period. To be able to offer a framework complete and exhaustive to the reader, the information used to draw up this work have been found from texts and articles in the original language and through the consultation of official web pages updated by the Ministry of Health of each State. The chapter that closes the first part of the thesis is based on building a comparison between the evolution of the NHS of each State and to present a summary of the new and newer regulations about health issued by governments. In the second part of the thesis, instead, the results of an empirical research are presented. They are carried out through in-depth interviews addressed to citizens of Mediterranean Welfare’s countries which have had experience of an urgent access to the NHS of another country that has the same type of Welfare. The goal is to offer a new point of view, based on the subjective perceptions of the interviewed with respect to the lived experience, and try to evaluate if from such experiences differences and/or similarities emerge that can provide useful information on the systems health care in each country. Considering the limitations of the research, in the first part they concern the continuous evolution of the historical and social landscape of each country and the consequent difficulty of being able to present a complete picture of the real health situation of a country. In the second part, the type of qualitative interview, the limited number of subjects interviewed, as well as the low availability of further empirical data do not allow to reach generalizable conclusions. The study concludes with some reflections that look, on the one hand, at the difficulty to provide a clear picture of the different national situations that are dynamic and in constant transformation; on the other hand, they focus on the possibility that the differences that emerged from the empirical investigation are not related to issues organizational and institutional architecture, as regards the perception of professionalism of health workers and their different way of acting the profession.

Nel presente lavoro si ha voluto fare riferimento ad una classificazione proposta nel 1990 da un famoso studioso danese, Gøsta Espring-Andersen, che suddivide l’Europa sociale in tre Regimi di Welfare (Liberale, Conservatore, Social-democratico), e alla revisione di tali Regimi effettuata da Maurizio Ferrera nel 2006. Espring-Andersen riesce a raggruppare e classificare il sistema sociale degli Stati europei, mentre Ferrera apporta in seguito una importante modifica alla classificazione per poter introdurre il concetto di Quarta Europa sociale, ovvero l’aggiunta di una nuova tipologia di Welfare, denominato Mediterraneo, che comprende Italia, Spagna, Portogallo e Grecia. È proprio sulla base di questa classificazione proposta da Ferrera che è stato possibile sviluppare il presente studio: Italia, Spagna, Portogallo e Grecia sono accomunate da una evoluzione storica e sociale molto simile che permette più facilmente di individuare similitudini e differenze utili per un paragone tra i SSN. Nella prima parte della tesi viene analizzata l’evoluzione storica e sociale di ciascun paese, dalla fine dell’Ottocento ai giorni nostri, rivolgendo particolare attenzione alle politiche sanitarie attuate in ciascun periodo storico. Per poter offrire un quadro completo ed esauriente al lettore, le informazioni utilizzate per stilare il presente lavoro sono state reperite da testi ed articoli in lingua originale e mediante la consultazione di pagine web ufficiali aggiornate dal Ministero della salute di ciascuno Stato. Il capitolo che chiude la prima parte della tesi è improntato a costruire un paragone tra l’evoluzione del SSN di ciascuno Stato e a presentare un quadro riassuntivo delle nuove e più recenti normative riguardanti la sanità emanate dai governi. Nella seconda parte della tesi vengono presentati, invece, gli esiti di una ricerca empirica effettuata tramite interviste in profondità rivolte a cittadini di paesi a Welfare mediterraneo che hanno avuto esperienza di accesso in urgenza presso il SSN di un altro paese che presenta la medesima tipologia di Welfare. L’obiettivo è quello di offrire un punto di vista nuovo, basato sulle percezioni soggettive degli intervistati rispetto all’esperienza vissuta, e provare a valutare se da tali vissuti emergono differenze e/o analogie che possano fornire informazioni utili sui sistemi sanitari di ciascun paese. Considerando i limiti della ricerca, nella prima parte essi riguardano la continua evoluzione del panorama storico e sociale di ciascun paese e la conseguente difficoltà di riuscire a presentare un quadro completo della reale situazione sanitaria di un paese. Nella seconda parte, la tipologia di intervista qualitativa, il limitato numero di soggetti intervistati, nonché la scarsa disponibilità di ulteriori dati empirici non permettono di arrivare a delle conclusioni generalizzabili. Il lavoro si conclude con alcune riflessioni che guardano, da un lato, alla difficoltà di fornire un quadro chiaro delle diverse situazioni nazionali che sono dinamiche e in costante trasformazione; dall’altro, si focalizzano sulla possibilità che le differenze emerse dall’indagine empirica non siano relative a questioni organizzative e di architettura istituzionale, quanto alla percezione della professionalità degli operatori sanitari e al loro diverso modo di agire la professione.

IL SISTEMA SANITARIO NAZIONALE NEI PAESI A WELFARE MEDITERRANEO: Un'analisi storico comparata e una ricerca empirica

Abstract

THE NATIONAL HEALTH SYSTEM IN MEDITERRANEAN WELFARE COUNTRIES: A comparative historical analysis and an empirical research
Nel presente lavoro si ha voluto fare riferimento ad una classificazione proposta nel 1990 da un famoso studioso danese, Gøsta Espring-Andersen, che suddivide l’Europa sociale in tre Regimi di Welfare (Liberale, Conservatore, Social-democratico), e alla revisione di tali Regimi effettuata da Maurizio Ferrera nel 2006. Espring-Andersen riesce a raggruppare e classificare il sistema sociale degli Stati europei, mentre Ferrera apporta in seguito una importante modifica alla classificazione per poter introdurre il concetto di Quarta Europa sociale, ovvero l’aggiunta di una nuova tipologia di Welfare, denominato Mediterraneo, che comprende Italia, Spagna, Portogallo e Grecia.
